from NBI- OFFICE, for the purpose of my employment in SOUTH KOREA.

I am providing a copy of my PASSPORT BIOPAGE that serve as my VALID ID and matches the signature
on this letter, with ID no. P7399051A issued by DFA TUGUEGARAO valid until MAY 30, 2028. Hereby
also attached the PASSPORT of MRS. EVELYN PANTALLON LORENZO as proof of her identity.

Any reliance I place whether knowingly or unknowingly on such material is therefore strictly at my own
risk. The representative is not liable for any legal issues, falsification, or nay damages ensuring in my
application for authentication and apostilled of documents at the Department of Foreign Affairs.

I agree that in the event the DFA found any falsification in my documents, I release the representative
from any liability and responsibility from mistakes, errors and correctness therein.
