    In connection with the registration of GARDEN VILLAS HOMEOWNERS ASSOCIATION (GVHA)


    located at The Yard, Garden Villas in Elenita Heights, Catalunan Grande, Davao City, the
    undersigned representative and on behalf of the incorporators thereof, hereby manifest our
    willingness:

    (a) to change its association name in the event another association has acquired a prior
    right to the use of the said name or one similar to it within the same Barangay, City
    or Municipality; and
    (b) to comply with all rules and regulations issued by the Department of Human
