I, SARBASISH CHATTOPADHYAY son of Late Kanai Lal Chattopadhyay, aged 59 years,

by faith Hindu, by occupation Service, by nationality Indian residing at 58/1 Sreerampur East,
Garia Park, Post Office – Garia, Police Station- Patuli, Kolkata-700084, do hereby solemnly
affirm and declare on oath that:
1. I say that in my Madhyamik Admit and Certificate my name is mentioned as SARBASHIS
CHATTERJEE son of Kanai Lal Chatterjee.

2. I say that the name SARBASHIS CHATTERJEE, SARBASISH CHATTERJEE and

SARBASISH CHATTOPADHYAY are the name of one and the same identical person, i.e.
myself.

3. I say that my true and correct name is SARBASISH CHATTOPADHYAY recorded in my

PAN and Aadhar Card issued by Government Of India.

4. I say that I am making this affidavit to produce before the concern authorities to prove that the
above mentioned names are my names.

That the above mentioned statements made in paragraph 1 to 4 are true to the best of my
knowledge and belief.
