Informed Consent Form for the Participants

PURPOSE OF THE STUDY

You are invited to take part in a research study. Before you


decide to take part in this study, it is important that you
understand why the research is being done and what your participation
will involve. Please read the following information carefully and
feel free to ask the researchers if there is anything unclear to you
or if you need more information.

The purpose of this study is to determine the potential


correlation between the STEM teachers’ attitude or character during
their respective classes and the motivation of FSUU Grade 12 STEM
students in learning their core subjects.

BENEFITS

You will gain no direct advantage from participating in this


study. However, the findings of this study based on your
participation will be helpful for the success of the research, as
well as for the respective entities that will benefit from the study.

STUDY PROCEDURES

You will be asked to participate in an online survey


on May 2, 2023 until May 7, 2023. The survey questionnaire consists
of three parts (3). All the expected questions in the survey are
presented below as the researchers would like you to read the
questions in advance and prepare yourself.

PART 1
Demographic Profile

Instruction: Please honestly provide the necessary information


about yourself for the following questions below.

PART 2
The Perception of Students Towards their
Teachers’ Character during Class Discussions

Instruction: Indicate your answer by selecting only one option


from all the given five-point Likert Scale:

1 - Never
2 - Rarely
3 - Sometimes
4 - Often
5 - Always

Note: Please be aware that the "teachers" mentioned in ALL of the


questions in this survey are referring to your subject teachers
in Basic Calculus, General Biology 2, General Chemistry 2, and
General Physics 2.
1. Our teachers are friendly whenever they interact with
students.
2. Our teachers talk enthusiastically about their subjects.
3. Our teachers have a sense of humor.
4. Our teachers are patient.
5. Our teachers are lenient.
6. Our teachers tell me in which areas I can still improve.
7. Our teachers give feedback on my strengths in the subjects.
8. Our teachers get angry quickly.
9. Our teachers are strict during class discussions.
10. Our teachers discipline harshly.

PART 3
The Perception of Students Towards the Specific Attitude that their
Teachers must Display in order to feel Motivated in Learning

Instruction: Indicate your answer by selecting only one option


from all the given five-point Likert Scale:

1 - Strongly Disagree
2 - Disagree
3 - Neutral
4 - Agree
5 - Strongly Agree

Note: Please be aware that the "teachers" mentioned in ALL of the


questions in this survey are referring to your subject teachers
in Basic Calculus, General Biology 2, General Chemistry 2, and
General Physics 2.

1. If our teachers talk enthusiastically in class, it excites


me to learn.
2. If our teachers smile at the class while talking, it
motivates me to listen during class.
3. If our teachers are patient, it motivates me to ask
questions regarding our lessons.
4. If our teachers are sincere in teaching, it makes me
understand the lessons more clearly.
5. If our teachers are friendly, it motivates me to participate
in class.
6. If our teachers have a sense of humor, it makes me more
involved in the classroom.
7. If our teachers are lenient, it makes me feel more
comfortable during class.
8. If our teachers show concern for our academic progress, it
motivates me to do better in my class.
9. If our teachers give feedback on my strengths, it helps me
to have better scores.
10. If our teachers constantly encourage us, it motivates me
to study better.

Once you agree to be a participant in this study, the


researchers will disseminate the online survey questionnaire to you
through your Facebook Messenger account or gsuite email.

If you consent to be a participant in this study, kindly tick the


“Yes” box:
YES, I consent to be a respondent.
NO, I do not want to be a respondent.

Duration. The participants will be surveyed depending on their


availability. Conducting the survey will take an estimated time of 3
to 5 minutes.

RISKS

The study procedures to be done may provide minimal risk to the


participants. Participants may find themselves challenged by the
questions to be asked. To minimize this risk, the researchers will
accept questions regarding clarifications from participants.

CONFIDENTIALITY

Your response will be reflected anonymously. Every effort will


be made by the researchers to preserve your confidentiality,
including the following:

1. Codes will be assigned to the participants and will be used on


all research notes and documents;
2. All collected data will be saved in a password-protected laptop
to ensure privacy and security;
3. All data will be deleted after research completion.

VOLUNTARY PARTICIPATION

Your participation in this study is voluntary. It is up to you


whether or not you decide to participate. If you decide to
participate, you will be asked to sign this consent form. After you
sign this consent form, you are still free to withdraw at any time
without giving a reason. Withdrawing from this study will not entail
any sanction, and will not affect the relationship you have, if any,
with the researchers.
