    office the documents needed for the 2021 BADAC Functionality Assessment and Audit, to wit:

    (1) Accomplished BADAC Self-Assessment and Audit Form;


    (2) Accomplished BADAC Data Capture Forms 1 to 6; and
    (3) Duly signed documents as MOVs.

    I further certify under penalty of law, based on information and belief formed after
    reasonable inquiry, the statements and information contained in these documents are true,
    accurate and complete.
