I, LINDA N. MANGUBAT, of legal age, married, Filipino,

and a resident of Brgy. Pallocan West, Batangas City, after
having been duly sworn to in accordance with law, do hereby
depose and state:

That I am the wife of LEONILO D. MANGUBAT who

works as a CONSTRUCTION WORKER;

That we are the parents of LIENELYN VIEL N.

MANGUBAT who is a high school student at Golden Gate
College, Batangas City;

That our daughter is applying for ESC SCHOLARSHIP

which is being offered by the above-mentioned school;

That I am executing this affidavit to attest to the truth of

all the foregoing facts, and for other legal purposes this may
serve.
