AFFIDAVIT OF LOSS

I, Eli Abad, Filipino, of legal age, single, and a resident of B15 Lot 23, Villa
Deludde, Subdivision, Lambakin, Marilao, Bulacan, after having been duly sworn in
accordance with law, hereby depose and say:

1. That I am a holder of an official receipt issued by ORNAP in relation to the

seminar we attended (44th Annual Convention and Scientific Meeting). To the
best of my knowledge the receipt has the following breakdown of expenses and
allowances:
*ORNAP FEE - Php4200.00
*FOOD ALLOWANCE - Php328.90
*TRANSPORTATION ALLOWANCE - Php1132.00
TOTAL = Php5660.90

2. That I used to put said official receipt inside our house;

3. That sometime on August 18, 2018, when I tried to look for the said official
receipt issued by ORNAP in the place where I usually put it, unfortunately I was
not able to find it;

4. That I have exerted all the efforts needed to find it but to no avail;

5. That the official receipt is now lost and beyond recovery;

6. That I am executing this affidavit to attest to the truth of the foregoing facts and
to use it for all intents and legal purposes.
