SUBMISSION OF SITA FORMS

(Surgery In Training Assessment)

It is of paramount importance in the interests of your own training, to ensure that the Surgery in Training
Assessment (SITA) forms are forwarded to the PGIM, on completion of assessments at the end of prescribed
training periods.

Therefore, you are required to make available with you the relevant set/s of SITA forms with its given colours,
while you are on your training in order to conduct the assessment by your Trainer/Supervisory Consultant.
These SITA forms are available on the PGIM website [pgim.cmb.ac.lk], for you to download/print.

Please follow the link https://pgim.cmb.ac.lk/sita/ to download/print the relevant SITA forms, in colour.
