Model Answers

Where was John and what was he doing before the seizure? Were any mood changes noticed? Were any
triggers noted? Tired, hungry or illness. Was there any warning? E.g. sensations, tastes or smells.

John had woken at 08.10 and was in the bathroom following his personal care routine.

John had bathed, brushed his teeth and was shaving at the time the seizure started, 08.35.

John appeared to still be tired and was quiet which is unusual for him in the morning.

There didn't appear to be any triggers or sensations before the seizure started.

Did John shout out? Did John fall? Did John go blank or stare? Was there any loss of consciousness? Was any
confusion noted?

John was blinking rapidly and then let out a high pitch scream, then fell to the floor and started
having a tonic-clonic seizure. John was unconscious during the seizure.

After the seizure John appeared confused and tired when I was talking to him, he didn't
remember waking up and having a bath.

Did John act in an unusual manner? Was there any sign of colour change? Any change in breathing? Were any
movements noted? Twitching or jerking. Did John fall or go floppy? Was John incontinent? Did John bite his
lip, tongue or cheek?

John was quieter than usual in the morning. I did not notice any change in his appearance or
colour, his breathing seemed normal but became loud and rapid during the seizure. John was
convulsing during the seizure and had a loss of bladder control. John also had some blood in
his saliva as he bit his tongue resulting in a minor injury.

How long did John’s seizure last? What happened after the seizure? Did John want to sleep?
How long did the recovery take? Was anything else noted?

John's seizure started at 08.35 and finished at 08.38 lasting 3 minutes and 18 seconds. John
was extremely tired after the seizure and wanted to go back to bed to sleep.

John spent 3 hours in his room sleeping, being monitored by staff. John then woke up and
appeared to have made a full recovery.

There were no other notes made.
