AFFIDAVIT OF INVESTIGATOR

I, Police Corporal Richard Gomez Lopez a bonafide member of the Philippine

National Police (PNP) presently assigned as Investigator at Nasugbu Municipal Police
Station with office address at Barangay Kanto, Nasugbu, Batangas, after having duly
sworn to and in accordance with law, do hereby depose and state the following:

At about 5:45 in the afternoon of January 21, 2023 at Barangay Palanan,

Nasugbu, Batangas, in connection with PDEA coordination number 10005-012023-
2546, under the direct supervision of PLTCOL MICHAEL FLORES DE GUZMAN, Drug
Enforcement Unit of this station led by PCPT ROMNICK CRUZ SARMIENTO
apprehended certain Alvin Patrimonio y Gutierrez alias Tangkad after selling one (1)
heat sealed transparent plastic sachet containing white crystalline substance believed to
be shabu amounting to five hundred pesos to the police officer who acted as Poseur
Buyer. Confiscated from the suspect possession and full control were; another one (1)
piece of heat-sealed transparent plastic sachet containing white crystalline substance
believed to be shabu, one (1) piece of five-hundred-peso bill with serial number
AZ164847 with initial markings RDD used as buy bust money. Suspect was arrested
and apprised of his constitutional rights. Alternative recording device was used during
said operation.

Pieces of evidence recovered;

* one (1) heat sealed transparent plastic sachet containing white crystalline
substance believed to be shabu bought from the suspect with markings on masking
tape marked as RDD-1 01-21-2023 with signature
* one (1) piece of heat-sealed transparent plastic sachet containing white
crystalline substance believed to be shabu recovered from the suspect with markings on
masking tape marked as RDD-2 01-21-2023 with signature
* one (1) piece of five-hundred-peso bill with serial AZ164847 with initial markings
RDD used as buy bust money marked as RDD-A 01-21-2023 with signature

That, I took pictures of the events such as the actual markings and inventory were
conducted at the placed of incident in the presence of Local Mediaman Paolo Avelino
Contis and Barangay Captain Herbert Bautista Martinez of Barangay Palanan, of this
municipality and the suspect himself.

That the undersigned was designated as investigator on case of the said

operation and prepared the following documents;
1. Request for laboratory Examination of confiscated suspected illegal drugs
2. Request for drug test for the arrested person
3. Affidavit of Investigator
4. Information sheet
5. Arrest booking sheet
6. Mugshot
7. Pictures

The suspect and confiscated two (2) pieces heat sealed transparent plastic sachet
containing white crystalline substance believed to be shabu with markings on masking
tape marked as RDD-1 01-21-2023 with signature and RDD-2 01-21-2023 with
signature was brought by the arresting officer at Batangas Provincial Crime Laboratory
Office for laboratory examination and drug test.

That anything not mentioned in this affidavit will be narrated during proper trial;

That I executed this affidavit to attest to the truthfulness of the above narrated
facts and to support the filing of appropriate charges against the above-named suspect.
