Kindly note the below terms and conditions for the gym subscription:

1. The above employee’s contribution will be deducted from your monthly salary over
12 months.
2. This is a 12 month subscription and it cannot be cancelled before that.
3. In case you resign before the completion of the above said 12 month period, you will
be required to pay the remaining amount from the company and employee’s
contribution.
4. In case the company terminates your services within this 12 month period, you will
be only expected to pay the remaining share from the employee’s contribution
amount.

The company will register your name with GymNation upon your acceptance of the above
terms and conditions.
