SUPPLY AGREEMENT

This Supply Agreement (hereinafter referred to as the “Agreement”) is entered by and

between: VIC YEE, of legal age, Filipino, married and a resident of Brgy. _______, _________,
Palawan hereinafter referred to as the “First Party”; and

RORY FUENTES, of legal age, Filipino, married and a resident of Brgy._____________,

__________, Palawan hereinafter referred to as the “Second Party”. They are collectively
referred to as the “Parties”, both of whom agree to be bound both by this agreeement.

AGREEMENT

A. PURPOSE. The parties have indicated an interest in forming and establishing a joint
venture exclusive purpose of quarry operation in Brgy. _________, __________, Palawan. The
operation shall do business under the name of the second party as the permittee.

B. CONTRIBUTION. The second party shall supply to the first party a sand and gravel
in the volume of FIVE HUNDRED CUBIC METERS PER YEAR.

C. PROFITS. That the first party shall pay the second party an amount of FIVE
HUNDRED FIFTY (P 550.00) PESOS, Philippine Currency, PER CUBIC METER.

D. TERMINATION. The violation of the terms of this agreement shall automatically

terminate this contract, and the respective parties shall return all the contributions that have
been made respective parties with corresponding legal interest without a need of demand.

IN WITNESS WHEREOF, parties herein affixed their signatures on _____ day of March,
2022 at Iloilo City.

ACKNOWLEDGMENT

BEFORE ME, a Notary Public, this ________ day of _________ 2022 at Palawan,
Philippines, personally appeared VIC YEE and RORY FUENTES with their competent
evidence indicated above and known to me and to me known to be the same persons who
executed the foregoing contract consisting of two (2) pages which herein party and their
instrumental witnesses have affixed their respective signatures and they acknowledged to me
that the same is their free and voluntary act and deed.
