I. Content Standards: The learner demonstrates understanding in


maintaining tools, equipment, and paraphernalia in
FOS.
Performance Standards: The learner independently maintain FOS tools,
equipment, and paraphernalia.
Learning Competency: Conduct self-evaluation on the required
performance.

Objective: Perform self-evaluation in the maintenance of FOS tools,


equipment, and paraphernalia using rubrics.

II. Topic: Evaluation of the performance in maintaining

Directions: Identify the following tools, materials and paraphernalia in the


front desk which are maintained regularly. Choose your answer
from the “Word Bank”. Write your answer on the space

FORM 1.1 SELF-EVALUATION CHECK

INSTRUCTIONS: This Self-Evaluation Check Instrument will give you


necessary information which is essential in evaluating
your current skills. Please check the appropriate box of
your answer to the questions below.

2.1 Clean the container regularly.

3.5 Sanitize the mouse to get rid of bacteria and viruses.

4.1 Check ink levels and refill immediately if it runs out.
4.2 For printer, avoid printing large volume of Materials.
4.3 Regularly check the ink levels.
4.4 Perform maintenance as scheduled.
5. Bills Detector
5.1 Avoid putting high volume of cash on it.
5.2 Use air compressor to blast dust on surface.
5.3 Use vacuum cleaner in every opening to remove debris from all
the rollers and sensors.
5.4 Use cleaning card for effective cleaning on the inside.
5.5 Cover with a clean cloth when not in use.
6. Card Verifier
6.1 Wipe the screen with a clean cloth.
6.2 Sanitize the keypad regularly.
6.3 Cover with a clean cloth to avoid dust build up.
6.4 Store in a cool, dry place when not in use.
7. Key rack
7.1 Sort the keys by room number.
7.2 Place not higher than eye level and never on the top of any
shelves.
7.3 Do not overcrowd shelves.
7.4 Get rid of the dust by using feather duster or clean cloth.
8. Cash Register
8.1 Remove and set aside the cash.
8.2 Vacuum out the dirt inside the drawer.
8.3 Vacuum on the keys and underneath them.
8.4 Sanitize the screen and keys regularly.
8.5 Do not wait for the paper rolls to run out.
8.6 Replace the ink cartridge when ink is fading.
8.7 Always close the drawer tightly.
8.8 Store at a room temperature at all times.
8.9 Cover with a clean cloth to avoid dust build up when not in use.
9. Guest Folio Rack
9.1 Every guest folio must have a reference number for easy
identification.
9.2 Arrange the guest folio in chronological order based on the
reference number.
9.3 Clean the folio rack with a clean cloth.
9.4 Store in flat and even surface.
10. Telephone
10.1 Place on a clean and flat surface
10.2 Avoid placing food and beverages near the telephone to avoid
spillage
10.3 Arrange the wires properly to avoid tangling
10.4 Wipe the dust with a clean piece of cloth
10.5 Regularly check if chords are attached properly
11. Calculator
11.1 Place on a clean and flat surface
11.2 Avoid placing food and beverages near the telephone to avoid
spillage
11.3 Keep inside the calculator case when not in use

12. Fax Machine
12.1 Wipe off the dust with clean rag at least once a week
12.2 Place on a secure place to avoid falling

13. Safety Deposit Box
13.1 Avoid encoding wrong pin several times
13.2 Always close the door tightly when not in use to avoid dust
build up on the inside
13.3 Protect from moisture for it will cause rust
13.4 Update the access code regularly for safety purposes

14. Log Book
14.1 Organize the log book in the rack
14.2 Label individually for easy identification
14.3 Remove dust by wiping with a clean cloth.
14.4 Arrange properly and keep inside the drawer or rack.
15. Room Key
15.1 Always check the marker if there is damage
15.2 Place on the assign room number to avoid swapping
15.3 Clean regularly to avoid rust with the use of a rust stain
removal
16. VingCard
16.1 Place in a key card holder to avoid scratches
16.2 Protect from moisture
16.3 Dispose damage card immediately to avoid inconvenience
17. White Board
17.1 Do not leave the ink on the white board for a long period of
time
17.2 Use a whiteboard marker only. Avoid using permanent ink
since it is hard to remove
17.3 Clean with a piece of cloth or eraser to get rid of the dirt
17.4 Use ink remover to remove stubborn ink
18. Envelopes
18.1 Arrange properly and store in a drawer
18.2 Avoid spilling liquid over it to prevent damage
19. Luggage Tag
19.1 Arrange neatly in a box
19.2 Keep in a cool dry place
19.3 Properly label the cabinet where it is stored to find it easily the
next time you will use them.
