Medical certificate. 11, Printout of Admission Confirmation Letter/PAO (Provisional Admission Order) 12. Recent taken colour photographs - 5 Passport Size and 5 Stamp Size (student have to upload the same in the Swagatham 23 portal). The originals of the following Mandatory Declaration/Undertaking forms duly signed are to be submitted during the verification process: 1. Declaration on Pending Documents submission. 2. Undertaking on Anti-Ragging This is mandatory as per UGC Guidelines. Undertaking on University Code of Conduct Compliance Students have to bring all the Original Documents for verification process, since there are chances that some students bring only the photocopies during the verification process.
