BASIC JOB STEPS POTENTIAL HAZARDS RECOMMENDATIONS

2.5. Dust Control 2.5.1. Provision of dust masks for workers and ensure
their use

3. Mobilize tools and equipment to worksite 3.1. Back injury (manual handling) 3.1.1. Manual handling shall be covered in daily TBT and
be implemented on site

3.2. Traffic and pedestrians 3.2.1. Audible reversing alarms shall be installed on
equipment/vehicles
3.2.2. Pedestrian safety precautions shall be discussed in
daily TBT
3.2.3. All traffic rules and signs should be followed (on site
speed limit is 25-km/h)
3.2.4. Only approved routes shall be used for access to

3.2.6. Trained banks-men with high-visibility vests shall
accompany the work activities near traffic/roads

3.3. Overturning of vehicle / Crash injuries 3.3.1. All drivers shall have valid driving licenses
3.3.2. The areas where heavy equipment are working shall
be clearly marked on site
3.3.3. Safe access shall be maintained at site for vehicles
and equipment
3.3.4. Use of seatbelts shall be mandatory

3.4. Slips / Trips / Falls 3.4.1. Housekeeping shall be discussed in daily TBT and
be implemented on site on a daily basis

3.5. Material Falls; Environmental hazard cause by Use Approved lifting equipment with daily checklist
equipment (operator’s inspection)
3.5.2. Use softener (protection pad) to protect the sling to
damage from sharp edge materials
3.5.3. Use slings and shackles of the correct SWL for
(Unloading of Pipes and materials) use of Lifting unloading. Lifting gear to have the current quarterly
equipment (Crane, Boom truck, Fork lift) colour code displayed
4. Pre-fabrication – Welding & Grinding of Steel Pipes 4.1. Fire and Burns 4.1.1. Inspected Welding machine (TAL- Equipment
Sticker) including all electrical cables by competent
person
4.1.2. Only Qualified Welder is required to the task
4.1.3. Use all the necessary PPE (Welding Gloves,
welding mask, welding apron) for welding works
4.1.4. No combustible material near the work place
4.1.5. Provide Container for welding butts
4.1.6. Fire blanket, fire extinguisher and a fire watcher if
necessary

4.2. Environment Hazard 4.2.1. Provide container can for all welding rod butts
4.2.2. Only approved equipment to be used
4.2.3. Report to GAMA HSE Department

5. Pre-fabrication - Bonding and Lamination of PVC 5.1. Environment Hazard 5.1.1. Required MSDS
Pipe and Fiberglass Pipe 5.1.2. Containment of all materials and chemicals as per
MSDS
5.1.3. Storage and provision of chemicals as per MSDS
5.1.4. Only trained or qualified person is allowed for
handling as per MSDS
5.1.5. Report all environmental spill to GAMA HSE

5.2. Health Hazard / Injury to health 5.2.1. Wear appropriate PPE as per MSDS
5.2.2. Only trained or qualified person is allowed for
handling as per MSDS
5.2.3. Restriction of the area by Barricade
5.2.4. Emergency First aid must be (Site Clinic)
6. Erection and installation of Pipe into excavation use 6.1. Falling materials, Environmental hazard 6.1.1. STARRT meeting to be conducted, form filled in and
of crane displayed at the working location,
6.1.2. Use Approved crane with 3rd party and site inspection
6.1.3. Use softener (protection pad) for slings if necessary to
protect from damage
6.1.4. All rigging/lifting equipment to be inspected and

Erection and installation of Pipe into excavation use 6.2. Crane Tilted 6.2.1. STARRT meeting to be conducted, form filled in and
of two (2) Cranes (Tandem Lift) displayed at the working location,
6.2.2. Use Approved cranes with 3rd party and site inspection
6.2.3. Use softener (protection pad) for slings if necessary to
protect from damage
6.2.4. All rigging/lifting equipment to be inspected and
display current colour code
6.2.5. Only trained riggers to work with the crane, tag lines to
be used to control the load
General Instruction:
- Only approved equipment to be used (crane, ,etc)
- No standing under a suspended load
After Field Welding

Piping and Field Joint Coating & Tape wrapping 6.3. Health Hazard / Injury to health 6.3.1. Wear appropriate PPE as per MSDS
(Fire Water Line) 6.3.2. Only trained or qualified person is allowed for handling
as per MSDS
6.3.3. Restriction of the area by Barricade
6.3.4. Available Emergency First aid (Site Clinic)
6.4. Environment Hazard 6.4.1. Provide plastic cellophane to protect the ground
6.4.2. Only approved PPE to be used/wear
6.4.3. Report to GAMA HSE Department
7. Radiography 7.1. Health Injury and environmental Hazards 7.1.1. 24 hours notice; Permit must be organized for inter
company concern
7.1.2. Early Preparation is necessary
7.1.3. Only Radiography group is allowed in the vicinity
7.1.4. Work area to be fully barricaded (nylon rope with
yellow and white warning tape), access restricted
authorized personnel only
7.1.5. Sign boards and flashing light must be on site.
7.1.6. Roving-men or a look out-man must be available at
site.
8. Hydro-testing (service test) 8.1. Injury cause by high pressure impact 8.1.1. STARRT meeting to be conducted, form filled in and
displayed at the working location,
8.1.2. Calibration of manifold and gauge
8.1.3. Approved connector and coupling
8.1.4. Test package must be available at site
8.1.5. Work area to be fully barricaded (nylon rope with re
and white warning tape), access restricted
authorized personnel only
8.1.6. Sign board required at Job Area

General Instruction:
- No defective tools are allowed
- No fabricated tools are allowed

9. Entrance to the trench to execute the work 9.1. Access across trenches 9.1.1. Provide access roads across the trenches

10. Demobilization 10.1. Work completion; Clutter/Rubbish 10.1.1. Workforce shall be alerted about the work
completion
10.2.1. Good housekeeping practice shall be implemented
throughout the day
