CATHETERIZATION: Under aseptic precautions, 7F sheaths were introduced into right


and left femoral vein using Seldinger’s technique, under general anesthesia. A 6F MPA
Catheter with a 0.032” J tipped Terumo wire was introduced through RFV into IVC-> RA-
>RV->MPA and PA pressures were noted. 6F MPA Catheter with a 0.032” Terumo wire
were advanced across ASD into RUPV. A 0.035” exchange length Amplatzer stiff wire was
exchanged for Terumo wire and anchored in RUPV. Another 7F MPA Catheter with a 0.032”
Terumo wire was introduced through LFV into IVC-> RA and the Catheter and Terumo wire
system was advanced across ASD into LUPV. 0.035” exchange length Boston scientific stiff
wire was exchanged for MPA catheter and anchored in RUPV. A 14 X 40 mm Sizing balloon
(via LFV access) was kept across the ASD for support while deploying the device. The right
groin was dilated with 16F dilator, following which 14F lifetech sheath was introduced over
the wire into RUPV, taking care to eliminate all air in the column. Using delivery cable, a 34
mm Amplatzer ASD DEVICE was advanced through the Lifetech sheath and deployed
across ASD Device. Device was prolapsing to RA through the defect. Hence device was
upsized to 36 mm Amplatzer ASD device. Post deployement, ECHO showed device in
unstable position, hence device was not released. She was shifted to the ward in stable
condition.
