Subject: Complaint Of Professional Behaviour and dental ethics

Dear Sir

I , Mrs. Samdana Shabbir, am your regular customer , our whole family taking dental treatment from
Fatima Memorial Dentistry over the years . Currently i am facing services and behaviour issues from
a Junior dentist Miss Neha (cell number :03224172215) she is student of Dr. Mujahid . Since the start
of December 2022, she is operating my root canal . She called me six times for treatment and where
she only made poor work of root canal .half of the root canal work done by Dr. Shehryar(PG) ,while
rest was looked by Mr. Mujahid. The thing that i had suffered for whole month is her behaviour .

She misbehaved various time and she never showed her professional ethics .She don’t know how to
treat a patient . from day first i was telling that i am not satisfied with her behaviour . But despite of
making bad treatment of my root canal she kept on instating that will treat.

please take my complaint at serious end because it will damage the image and goodwill of Fatima
