Clinical
Information
Practical
Advice
for
the
Use
of
Low
Level
Laser
Therapy
Cleaning
the
skin
with
isopropyl
alcohol
will
diminish
any
reflection
of
the
laser
beam
by
the
tissue
surface
due
to
natural
oils.
In
the
non-‐contact
method,
a
fairly
large
percentage
of
the
laser
beam
may
be
reflected
by
the
stratum
corneum,
thus
cutting
down
the
available
penetration
power
of
the
LLLT
beam.
The
further
the
beam
is
away
from
the
target,
the
greater
the
divergence;
thus
the
less
light
is
available
for
treatment.
Therefore,
the
laser
should
be
used
in
the
contact
method.
(i.e.:
laser
device
is
placed
directly
on
the
skin
surface)
There
are,
of
course,
some
exceptions
(i.e.:
in
treating
burns,
where
the
tissue
surface
is
extremely
sensitive).
In
order
to
place
the
origin
of
the
beam
closer
to
the
target
tissue,
the
probe
should
be
applied
with
some
pressure
(i.e.:
compression
of
subcutaneous
fat
and
blood
vessels,
removing
possible
absorbers
such
as
haemoglobin)
Start
a
treatment
with
a
low
dose
of
laser
energy,
and
then
continue
with
the
minimal
dose
that
will
achieve
a
therapeutic
response.
Finally,
increase
the
dose
every
three
(3)
treatment
sessions,
only
if
the
treatment
is
not
obtaining
the
desired
results.
Karu
and
Mester
stress
that
a
treatment
two
(2)
to
three
(3)
times
per
week
achieves
the
most
positive
response.
With
high
cumulative
doses,
one
tends
to
treat
less
frequently
than
using
low
total
energy
densities.
There
is
a
general
belief
that
to
treat
too
often
is
neither
desirable
nor
necessary.
LLLT
has
a
cumulative
effect
with
each
additional
treatment.
Treatment
Basics
Caution
The
length
of
treatment
and
power
settings
detailed
are
recommendations
only.
The
choice
of
all
treatment
variables
is
ultimately
the
responsibility
of
the
practitioner.
The
receptivity
of
the
patient
and
the
nature
and
specifics
of
the
ailment
are
always
unique
to
each
individual
case.
After
several
applications,
the
practitioner
should
begin
to
achieve
a
working
knowledge
of
the
appropriate
dose
required
for
their
specific
clientele.
General
Protocols
a) Degrease
the
surface
of
the
skin
with
isopropyl
alcohol
to
reduce
surface
skin
reflection
if
the
surface
is
oily
or
greasy.
b) Clean
the
surface
of
the
laser
probe
tip
with
isopropyl
alcohol
to
sterilize
it.
Always
disinfect
the
probes
after
each
treatment
using
isopropyl
alcohol.
Use
a
swab
to
clean
the
area
around
the
laser
diode
but
please
take
care
when
doing
so
as
the
laser
diode
lens
is
fragile.
c) Always
protect
the
patient's
eyes
with
laser
glasses
from
direct
or
indirect
contact
with
the
laser
while
treating
around
the
proximity
of
the
face.
d) Always
treat
joints
by
irradiating
deep
in
the
joints.

e) Avoid
letting
the
laser
probe
contact
an
open
wound.
Cover
the
wound
with
saran
wrap,
plastic
film
or
a
sterile
bandage
to
protect
the
laser
probe
during
treatment
and
help
to
prevent
cross
contamination.
f) For
wounds
always
treat
the
periphery
of
the
wound
first
and
then
the
surface
area
of
the
wound
(wound
bed).
g) In
some
cases,
pain
may
increase
for
the
first
couple
of
treatments,
or
4
or
5
hours
after
the
treatment
but
will
regress
progressively
with
each
subsequent
treatment.
This
phenomenon
may
be
a
result
of
the
laser
energy
“re-‐awakening”
the
nerve
endings
in
the
wounded
area.
h) If
one
or
several
painful
points
still
persist
after
treatment
completion,
you
may
retreat
these
regions
as
previously
described.
i) Due
to
the
sensitivity
of
burns,
hold
lightly
on
the
wound
or
scan
at
a
distance
of
2
to
3
mm
from
the
affected
tissue.
j) Laser
therapy
can
be
used
by
itself
or
in
conjunction
with
any
other
pharmaceutical,
modality
or
other
treatments.
Note:
• Certain
pharmaceuticals
are
light
sensitive
and
laser
irradiation
may
increase
or
decrease
their
effectiveness
in
the
body.
• Ask
your
patients
about
the
sensations
they
experience
during
and
after
treatments.
It
will
provide
you
with
valuable
knowledge
of
treatment
protocols
and
treatment
efficacy
to
apply
in
future
treatments.
Acute
Cases:
• Treat
daily
for
the
first
three
to
five
days
and
then
treat
3
times
per
week
until
the
condition
is
resolved.
Conditions
are
typically
resolved
in
~
6
treatments
• The
more
recent
the
injury
the
more
effective
the
laser
therapy
will
be
in
reducing
inflammation
and
increasing
mobility
Sub-‐Acute
Cases:
• Treat
3
times
per
week
for
3
weeks,
then
2
times
per
week
until
the
condition
is
resolved
• Conditions
are
typically
resolved
in
~12
to
15
treatments
• If
there
is
no
perceivable
improvement
after
the
third
treatment,
increase
the
energy
density
by
increasing
the
length
of
treatment
Chronic
Cases:
• Treat
2
times
per
week
until
the
condition
is
resolved
• Don't
expect
results
too
quickly,
continue
treatments
until
the
20th
to
30th
session
for
maximum
results
• Commence
treatment
with
the
most
painful
areas
first
• Each
successive
treatment
should
demonstrate
an
improvement
over
the
previous
treatment

Contraindications
1) Do
not
treat
the
abdomen
of
a
pregnant
woman.
2) Do
not
treat
any
types
of
neoplasia,
cancer
or
cancerous
lesions,
(benign
or
malignant).
Cautions
in
the
use
of
Laser
Therapy
1) Within
½
inch
of
the
eye
structure.
Never
point
the
laser
in
the
direction
of
any
person’s
eyes.
2) Do
not
treat
endocrine
glands
such
as
the
pancreas
and
the
thyroid.
3) For
a
new
patient
or
a
patient
who
may
be
light
sensitive,
spot
test
the
laser
on
an
area
of
tissue
to
determine
if
they
are
adversely
sensitive
to
laser
treatment.
4) If
a
patient
is
currently
prescribed
photosensitive
medicine,
NSAIDS,
steroids
or
corticosteroid
medicine,
try
to
allow
2
weeks
as
a
wash
out
period
prior
to
laser
treatment.
If
a
wash
out
period
is
not
possible,
reduce
the
laser
power
to
½
of
the
recommended
power
indicated
and
increase
with
each
treatment
as
the
patient
reliance
on
photosensitive
medicine,
NSAIDS,
steroids
or
corticosteroid
medicine
reduces.
5) Start
treatment
at
the
most
painful
area
first
and
follow
the
treatment
specifics.
Treat
surrounding
tissue
next.
6) Under
no
circumstances
should
the
patient
feel
anything
more
than
a
slight
warming
effect.
If
the
patient
feels
hot
sensations,
pain
or
burning,
STOP
LASER
TREATMENT
IMMEDIATELY
and
check
laser
equipment.
If
laser
probe
head
is
hot
to
the
touch,
call
Theralase
Inc.
immediately
for
service.

7) Under
no
circumstances
over-‐treat
an
individual
as
over-‐treatment
may
lead
to
bio-‐inhibition
