    Subject: Request for Road Right of Way Clearance

    Dear DE Bermal,

    I am writing to request for a hi-way clearance in relation to my Building Permit Application pending
    before the Office of the Building Official in the municipality of Labo.

    The applied commercial structure is located along the hi-way of Pinagtambangan (Talobatib). I
    attached here the following requirements for your reference:

    1. One (1) machine copy of Transfer Certificate of Title (TCT)


    2. One (1) machine copy of Tax Declaration
    3. One (1) blue print of Lot Plan/ Survey Plan
    4. One (1) set of Building Plans signed and sealed by licensed professionals
    5. One (1) machine copy of cadastral survey portion
    6. One (1) copy of Notarized Affidavit of Undertaking
    7. PENRO Certification
