the-job training to their company. To all staff and doctors of radiology
department, who had played a major part in the acquisition of my training, for

her necessities, her work immersion voyage would not be a tremendous

BIOGRAPHICAL SKETCH
The author, Angel M. Cuevas, was born on March 19, 2005, in PGH
Paete Laguna. She is the oldest among the two children of Ms. Maricel M.
Cuevas. She completed kindergarten at Day Care Center at Paete, Laguna,
and graduated with academic excellence. Then she continued her education at
Quinale Elementary School, she completed her secondary education in Liceo
De Paete. She is now pursuing her Senior High School at Paete Science and
Business College, Inc.
She enjoys baking and playing volleyball. She has always been attracted
to purple and enjoys watching vlogs. Her greatest ambition is that she wanted
to have her own bake shop. She was also looking forward to realizing her life's
dreams and goals with the help of Almighty God.

EXECUTIVE SUMMARY
This narrative report from Laguna Holy Family Hospital, Inc.'s Work
Immersion program aims to provide an executive summary. The narrative
report's first portion includes information about Laguna Holy Family Hospital,
Inc. I was assigned to the radiology department, but before we were sent there,
Ms. Abby gave us an orientation where she explained what we should and
shouldn't do. It wasn't a strict seminar, and when we went to each department,
I introduced myself. At first, it was awkward, but after a while, it became okay
because I wasn't as shy, and then they were nice when we had free time, and
we grew closer. Ultrasounds are not permitted, we were watching if there is no
doctor, we will arrive at the dormitory at 10 a.m. After that, throughout the 15
days I was there, all I did was print the ultrasound results, present them to the
patient, and then call another patient. I was extremely satisfied with this. First
of all, returning home at night after an early morning shift were one of the things
that exhausts us; otherwise, we like our work too much at the Laguna Holy
Family, Inc.
Work Immersion training took place from March 13, 2023 to March 31,
2023. It lasts 15 working days or 120 hours. The narrative report depicts
ordinary daily activity. I was given routine tasks that I was already familiar with,
although some of them helped me develop my talents. The narrative highlights
all achievements as well as newly learned and enhanced abilities like
communication and time management for self-evaluation. Making the most of a
work immersion requires not only learning new things, but also resolving issues

that comes up while you're doing the work immersion. In the narrative report, I
will explain how, as a competent trainee, l overcame challenges and made
adjustments. I fervently recommend and suggest that new student-trainees
perform tasks Laguna Holy Family Hospital, Inc. hosts on-site training. This
narrative report also includes the appendices, my work immersion photos, and
the certificates that attest to my time spent working at Laguna Holy Family
Hospital, Inc.

Chapter 1
INTRODUCTION
Work immersion gives Senior High School students the chance to
become familiar with the workplace, simulate employment, and put their skills
in areas of specialization or applied topics in an actual work setting in order to
prepare them for all curriculum exits. Work immersion must be first-hand in
order to give students a chance to improve themselves by experiencing the real
world.
This will enable learners to decide what course they want to attend in
college as a Grade 12 student. Work Immersion is an opportunity for students
to develop professionally by simulating real-world employment. Additionally,
this program offers chances to experience a real-world methodology of a
specific. using real equipment, machinery, and papers.
After our work immersion, we are required to write a narrative report in
which we must evaluate the knowledge and skills we have learned. We can
choose the course or strand that best suits our personalities and ability levels
using the results of this assessment. Therefore, requiring each student who
participates in work immersion to submit a submission that must be approved
by their class adviser after being checked and reflecting on how our work
immersion experience benefitted us.

Historical Background of the Company
History of Laguna Holy Family Hospital Inc.
The Holy Family Clinic (HFC) started its operation on November 14,
1970, and was considered the first private hospital established in the 4th district
of Laguna. It was managed by a husband-and-wife team of Drs. Eli Sr. and
Alejandra Malaya. The hospital was classified as a secondary-level private
hospital with a 15-bed capacity. In the mid-70s, the Holy Family Clinic bed
capacity was increased to 25 and peaked at 40 beds by the end of the '70s. At
this time, an additional building was constructed to accommodate suite rooms
and ward beds.
At the turn of the 80s, other private hospitals were established in the
area. In addition, private clinics were sprouting. As a result, the occupancy rate
of the hospital dropped significantly. Compounding the problem is the
government and Medicare requisition of having a 1:5 nurse-patient ratio. By
mid-80 some hospital wings with patient rooms were closed, decreasing the
hospital bed capacity to 25 then 15, and finally to 12 beds at the end of the '80s.
Through the 90s and the turn of the century, hospital operation remains at a
stable level.
On September 17, 2002, the Laguna Holy Family Hospital under the
ownership of Holy Family Health Service Inc., was established; composed of
practicing Medical Doctors patronizing the hospital. Overall operation and

management were handled by the corporation and with it, the activities of the
hospital became "robust" to say the best.
In 2007, the operation of 3 stations dialysis unit was started and is known
as the first dialysis center in Sta. Cruz, Laguna.
As time goes on, with the noble purpose of providing excellent,
compassionate medical services to our community, the Laguna Holy Family
Hospital (Holy Family Health Services Inc.), has decided and begun the
construction of a 5-storey building which is now known as Annex Building A.
The said building was inaugurated on November 2009. In 2014, a 4-storey
Building known as Annex B Building was established and the very first Eye
Center in District 2 of Laguna started its operation with state-of-the-art
equipment;
During the LHH 12th Foundation Anniversary Celebration on October 7,
2014, the Laguna Holy Family Hospital launched the Dr. Eli Malaya Sr. Lecture
Series. The activity was organized and designed by Nursing Service
Department and has continuously been done to date.
On January 2015, The Laguna Holy Family Hospital started to implement
hospital departmentalization wherein department heads in Medicine, Surgery,
Pediatrics, OB-Gynecology, and Anesthesia were appointed. Subsequently, in
July of the same year, the Laguna Holy Family Hospital was awarded by the
Department of Health as a Level 2 General Hospital with an authorized 71-bed
capacity. Other services such as Clinical Laboratory (tertiary), Blood Station,
Pharmacy, Dialysis Unit, Specialized Diagnostic X-Ray services (Level 2), and

Computed Tomography were issued a license to operate.On January 2016, the
Hemodialysis Units has increased its stations to 18 to serve the overwhelming
rise in the number of patients needing hemodialysis treatment. Aside from being
recognized by the DOH as a level 2 Hospital, on October 2017, the Laguna
Holy Family Hospital has been conferred with International Organization for
Standardization (ISO) 9001: 2015 certification, for conforming to quality
management system standards. ISO certification attests that a management
system, processes, services, or documentation procedures have all the
requirements for standardization and quality assurance. ISO is an independent,
non-governmental, international organization that develops standards to ensure
the quality, safety, and efficiency of LHFH services and system.
On April 2018, the Holy Family Hospital Health Service Inc. has been
amended and renamed Laguna Holy Family Hospital Inc., This was also the
year when the 5-storey building known as Annex Building C was constructed.
In the first quarter of the year 2020, with the overwhelming number of COVID-
19 patients not only in our country but worldwide, the hospital immediately
constructed St. Joseph Wing, a building that caters moderately to critically ill
Covid-19 patients. A molecular Laboratory was then established and
recognized by the Philippine government through the issuance of DOH-License
to Operate.
In December 2022, the Department of Family and Community Medicine
was founded thru the efforts of the hospital board of directors spearheaded by
Dr. Baldovino, Dr. Ramos, and Dr. Malaya in coordination with the
Department's core Consultants headed by Dr. Ronaldo Lapitan. Along with the

recognition of the Department, a Traditional or Hospital-based Residency
Training Program was also established in response to the call for the
implementation of the Universal Health Care Law together with the hospital's
commitment to provide continuous quality care for patients from womb to tomb.
Even the pandemic brought by the COVID-19 surge did not stop the
Laguna Holy Family Hospital Inc., from achieving its objectives, goals, vision,
and mission hence continuously making ways to serve the community with
compassionate health care services and facing challenges with enthusiasm.
The Laguna Holy Family Hospital Inc. remains a healthcare institution where
history continues to unfold.

The radiology department at Laguna Holy Family Hospital is open 24
hours a day, 7 days a week, although CT scan and biopsy procedures are only
permitted when performed in conjunction with ultrasounds, which are typically
performed by doctors.

passed the board exam on 2016 and he is the chief of the radiology department
or the highest of them all that when someone abuses, he is the first to be told
so that sir jm can list everything.

DAILY ROUTINARY ACTIVITY
In this chapter, the student trainee presents the day-to-day standardized
actions or procedures that are followed regularly and often repetitiously.

EXPERIENCE AND JOB DESCRIPTION
March 13, 2023 Ma'am Abegale at Laguna Holy Family Hospital,
(Monday)
Inc. was in charge of our orientation, I arrived early
on my first day and waited for her. Ma'am Abegale
Villaran is the hospital's assistant for human
resources, she said as she began to introduce
herself. She then went on to discuss the appropriate
clothing they need, including a white t-shirt, slacks,
and school uniform. Additionally, wearing anything
skimpy is not allowed inside the hospital. She also
spoke about our time inside and outside. The most
important thing is to not be late.
March 14, 2023 What I did was print out the results of the ultrasound
(Tuesday)
and the patient called me, then we met with different
doctors at the hospital, but in one day there were
two doctors and today, Doc Merca and Doc Silo
were very kind and loving to their employees and

when all our patients are done, we are waiting to go
home and we were just letting the time pass until it’s
5pm so that we could go home.
March 15, 2023 Just like that again, I was printing the result and the
(Wednesday)
patient called me back in one day and we only work
for more than 5 or 6 hours because the doctor's
appointment in the morning would start at 10 and
ends at 12:40 then we have lunch for 10 mins, then
1:30 to 2:30 we were not able to do the ultrasound
because there was no available doctor at that time.
March 16, 2023 Just like that, I was printing again and the patient
(Thursday)
called me, then Ms. taught us how to do the
ultrasound, what she did was show us the right and
left liver and the right liver is bigger than the left one
and we learned a lot from Ms. Lerisa. We were
waiting again for 5pm to go home because we don't
have anything to do anymore, we just bonded and
watched as they wandered around the hospital,
they took us on a tour to get tissues because we ran
out of tissues, then when we get back to the
department.
March 17, 2023 Day 5 we didn't do anything because there was no

doctor at that time so we just cleaned and changed
tissues then we cleaned the whole radiology

department then bonding together with the others
and that's how we all became very close at that time
and we were open and there was no shame, the
doctor's assistant told us about her life and doc silo
was always joking with us, it was so much fun in the
radiology department every day
March 20, 2023

March 21, 2023 So, on day 6, doc Merca is our doctor for this day,
(Tuesday)
he had a biopsy and I watched, he showed me how
he had a biopsy, then when he did the biopsy, when
he was done, he gave us some foods in the
radiology department, he gave us all free lunch, he
says that he always exempts everything. Doc Merca
is also very kind. He also told a story about his
experience while studying and becoming a doctor
and because of them I was very impressed with
what they went through
March 22, 2023 There was a printout and the patient waiting called
(Wednesday)
me again to the doctor because it's not possible to
do an ultrasound without a doctor, so we didn’t do
anything, that's all. Sometimes we go around the
whole hospital and then we go to the ICU with doc
Cayos too. Doc Cayos is kind, every friday he is the
only one who has a lot of patients, too many

printouts are being made and doc Cayos became
close to me because he is very kind
March 23, 2023 This day went on the same as yesterday, waiting for
(Thursday)
the doctor, then all the patients will be listed so that
they will be called when it’s their turn to see the
doctor, now there are only a few patients, so we can
rest for a long time and we can clean the
department and after work, I will accompany ms.
Lorrie in the ICU to see the patient of doctora
Cayos, we got another paper from the second floor
for the in-patients for the ultrasound. I fell asleep
because our patient was done and we were a bit
tired and waited until we can go home because we
are not allowed to go before the time is over
because we need at least 9 hours in a day. After the
work is done, we just stay in the department or just
sleep
March 24, 2023 Doctor Merca is the most hardworking doctor

because he always gets in. Ms. Lorrie said that
everyone who doesn't get in is the doctor who gets
caught, so it seems like the doctor is always there.
After that, I fell asleep again because we were done
assisting the patients and we were a bit tired and
waited until we can go home again. That's it again,

waiting for the doctor, then all the patients will be
listed so that they will be called when the doctor
comes, then now there are only a few patients, so
we can rest for a long time and we can clean the
department, then we will go to the ICU again to
check the Doc Merca's patients are always free in
the radiology department, so it's too much fun in the
radiology department.
March 27, 2023 Day 10, the same thing happened again, we waited
(Monday)
again for the doctor, when they came, the
ultrasound was done and I was the one who printed
it and the patient waiting called me again to the
doctor because it is not possible to do an ultrasound
without one, so we are not doing anything when
there is no doctor, it's just like that, sometimes we
go around the whole hospital and then we go to the
ICU with doc cayos,, a lot of prints are being made.
One of our red tech colleagues, sister Nicole, is the
one who is leaving to go to another band so that we
can all work there, which is sad even for me
because she is one of the closest to me there, sister
Nicole even Ms. Lorrie I know that's his best friend
March 28, 2023 The patient immediately called me once the result
(Tuesday)
was printed again, Ms. Lerisa then taught us how to

perform an ultrasound. We demonstrated to her
how the right liver is larger than the left, and we
learned a lot from her. We waited once more for
employee number five to leave because we had
nothing else to do but bond and observe them as
they wandered around the hospital in search of
tissues because we had run out, we fixed it for
tomorrow and it will be ready when we get back to
the department.
March 29, 2023 There are only few patients left, so we can rest for
(Wednesday)
a while and clean the department. After work, I'll go
with Ms. Lorrie, to the ICU to see the patient of
Doctora Cayos. After that, we got another paper
from the second floor for the in-patients for the
ultrasound. I fell asleep again because I was so
tired. I've been working for nine hours, and I'm still
in the department, so I have to go to the toilet.
March 30, 2023 Once the result was printed out once more, the
(Thursday)
patient called me right away, and Ms. Lerisa
showed us how to use an ultrasound. She taught us
a lot. While we once again waited for employee to
exit because there was nothing else for us to do but
bond with them and watch them look around the
hospital for tissues because we were out, we fixed

it for tomorrow and it will be ready when we return
to the department.
March 31, 2023 That's it again, waiting for the doctor, all the patients
(Friday)
will be listed so that they will be called when it’s their
turn to see the doctor, there weren’t as many
patients like the last time, so we can rest for a while
and clean the department, and after work, I'll go with
Ms. Lorrie to the intensive care unit to see the
patient of Doctora Cayos. After that, we got another
paper from the second floor for the in-patients for
the ultrasound, and then I fell asleep because
April 1, 2023 Today is my last day and just like that, there was a
(Saturday)
printout, and the patient waiting called me back to
the doctor because it is impossible to perform an
ultrasound without one, so that is all we do. Then,
occasionally, we took a tour around the entire
hospital before visiting the ICU with Dr. Cayos
because despite his kindness and the fact that he is
the only one with a lot of patients on Fridays, too
many prints are being made, and as a result, Dr.
Cayos and I became good friends and doc cavos
found out that it was my last day, he made milk tea
and fed us on our last day of work immersion at the
hospital because they said they would miss us so

much, even I was sad that I would not be traveling
early and would be coming home late and it ended
early. We took pictures together with the other
staffs.

Chapter 3
SELF-ASSESSMENT
In this chapter, the student trainee comprehensively self-assessed the
additional knowledge and skills achieved during the training program.
Achievements
a. Skills Learned/Enforced
The trainee acquired a number of skills including communication
abilities, which are crucial for providing patients with quality care. She
gained knowledge on how to interact with patients, their families, and
other healthcare professionals in a clear and effective manner. She
also learned effective time management skills. Hospitals may be
highly busy places, and as a trainee, you'll need to learn effective
time management skills to make sure all assignments are finished on
time.
b. Equipment, Machinery, Testing Apparatus, etc. Handled
The trainee was not permitted to utilize certain of the equipment
in the Radiology Department. She only permitted handling of the
printouts. She also managed the Outpatient Releasing Logbook
and printed the ultrasound data.

c. Best Experience on the Job
The best experience is the one to print the ultrasound and assist
in the CT scan and in going to the ICU and call the patient in the daily
work and the one to meet a new friend.
Faults
a. Causes
Because there were so many patients, she sometimes forgot
to give the results in the other room.
b. Suggested Solutions
What should always be done is when you print it, write it on
paper and call a doctor, then immediately give it to the other room
so that the patient can be given the result.

Chapter 4
RECOMMENDATIONS
In this chapter, the student trainee shares insights toward how the on-
the-job training incorporated to STEM of PSBC alternative solutions to have a
better company management and advice to the upcoming student-trainees who
will conduct work site training at the same company.
Potential of the Company as a Training Ground
For all new trainees, The Laguna Holy Family Hospital, Inc. is a top-notch
training center. The company's staff treated the trainee like a fellow employee.
The organization is able to give the trainee useful experience as well as new
information and abilities for working in a hospital.
a. Availability and Appropriateness of Training, Facilities,
Equipment, Tools and Machinery
Training, facilities, tools, and machinery are all available and
appropriate for the task at hand for the student trainee. The facility is
equipped with all the necessities. Equipment, tools, and machinery
owned by the company offer further knowledge and practical skills.
b. Company Cooperation Personnel
The company can help the student in training to become familiar
with the real cooperation in the work with other staff is in accepting
the patient we are taught to be patient with different patients.

Advice to Future Student-Trainees
Here are some suggestions to help you succeed as a student-trainee if
you are thinking about applying to be one in a hospital. Keep an eye out for
