JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

WE, EDWIN D. PERALTA, JR. and LAURENCE MATIBAG, both of

legal age, Filipino, and residents of Brgy. Solo, Mabini, Batangas, and Brgy.
Bagalangit, Mabini, Batangas, respectively, after having been duly sworn to
in accordance with law hereby depose and state:

That we personally know DARWIN DE VILLA ORTEGA because we

have been close friends for a long period of time up to the present;

That in the Certificate of Registration of his motorcycle, his surname

was mistakenly entered as ORIEGA;

That his true and correct surname is ORTEGA as indicated in his

Certificate of Live Birth as attached;

That we declare that the true and correct surname of DARWIN DE

VILLA ORTEGA is ORTEGA and not ORIEGA;

That we are not related by affinity or consanguinity to DARWIN DE

VILLA ORTEGA; and

That we are executing this affidavit to attest to the truth of all the
foregoing facts and circumstances, and for whatever legal purpose this may
serve.

IN VIEW THEREOF, we have hereunto set our hands this 28th day of
December, 2021 in Batangas City, Philippines.
