Examples of behaviors to support my self assessment: I have recognized holes in my learning and I have brought them to attention in the moment or after at a
more convenient time for explanations. I have attempted to compile the available information, but I still need to work on this to be a more effective SPT.

Regarding this Professional Behavior, I would like to improve in the following ways: In the treatment and evaluation sessions I need to better articulate my ideas
and to do more research on my own time about the knowledge/literature/evidence to have a better understanding of the concepts and how they can be
implemented in the clinical setting.

Examples of behaviors to support my self assessment: Throughout treatment and evaluation therapy sessions I have been able since the midterm to progressively

critically analyze the information that I have been obtaining from the patient presentation in a simpler and more foundational format. I have felt more challenged to
think in the moment since the midterm as to future planning and what would be the next most logical step to take along with still understanding the holes in my
knowledge that have been able to at least partially fill with supplemental evidence-based research.

Regarding this Professional Behavior, I would like to improve in the following ways: I still need to in the evaluation sessions to better articulate my ideas that can
be supported with evidence-based research in the moment so I can better explain to patients. Since the midterm, I have been and will continue my own time
further research on increasingly more complex patient presentations to gain more than foundational knowledge/literature/evidence to have a better understanding
of the concepts and how they can be implemented in the clinical setting.

Examples of behaviors to support my self assessment: I have been reviewing what I have written in the documentation to check for spelling and grammar
issues while also asking for assistance if I am unsure about the phrasing or if it is in the relevant location. I have only used the computers appropriately at the
clinic to review and document information. I have continued to monitor my verbal and nonverbal communications to remain respectful and professional while
in the treatment and evaluation sessions.

Regarding this Professional Behavior, I would like to improve in the following ways: I need to work on my confidence when I am speaking with patients by

doing more research so I know what I am talking about and conversing more with the patient to build up their trust and confidence in me as a SPT.

Examples of behaviors to support my self assessment: Since the midterm I have been more confident and comfortable when I am communicating with various
patients in the daily treatment sessions and evaluations to gain the appropriate information required while also gaining the trust of the patient. I have
continued since the midterm reviewing what I have written in the documentation to check for spelling and grammar issues while also having to ask less often
for assistance if I am unsure about the phrasing or if it is in the relevant location. I continued since the midterm to only used the computers appropriately at the
clinic to review and document information. I also still since the midterm have continued to monitor my verbal and nonverbal communications to remain
respectful and professional while in the treatment and evaluation sessions.

Regarding this Professional Behavior, I would like to improve in the following ways: I still need to work on improving my utilization and modification of my
communication in verbal and non-verbal forms of communication like visual cues to meet the needs of different patients mainly with cognitive deficits like post
stroke or TBI patients.

Examples of behaviors to support my self assessment: I have been able to look through the computer to pull up different information that I need for the
treatments or evaluations with some assistant if I’m not sure where I should go next.

Regarding this Professional Behavior, I would like to improve in the following ways: I will be more observant and using my critical thinking in the future to
better see any potential issues that my arise and what I plan to fix them.

Examples of behaviors to support my self assessment: Since the midterm I have been able to continue to use the EMR to discover various information
required for documentation like imaging or some background information from a previous visit with less assistance required. I have been able to recognize
more of the more frequent problems associated with working with a particular patient presentation and how to best solve the problems before and during their
occurrence, but I am still having some difficulty with the more complex cases as my knowledge based is not as advanced for the high complexity. I have been
able to come up with some simple variations to a treatment if the patient is not able to get in a desired position, desired equipment, or machine is unavailable
at the desired time

Regarding this Professional Behavior, I would like to improve in the following ways: I will continue to have various options near by that could solve a potential
problem that could be foreseen or at least a backup plan if the treatment does not go to plan for unforeseen circumstances as to improve my efficiency and
effectiveness in the moment

maintain the confidentiality as I have not discussed any information with individuals outside the patient’s care. I have not let my personal beliefs enter into
the discussion with the patient at any point while also getting the information required for the documentation.

Regarding this Professional Behavior, I would like to improve in the following ways: I would like to do some more background information so I can be
more confident when I am talking with the patient if they have a particular question about an intervention or about their condition.

Examples of behaviors to support my self assessment:Since the midterm I have been able to talk to all of my coworkers and clients in a respectful and
appropriate manner that maintain the confidentiality as I have continued to not discussed any information with individuals outside the patient’s care.
Since the midterm along with communicating with various patients in the daily treatment sessions and evaluations I have been able to progressively gain
the appropriate information required while also gaining the trust of the patient from evaluation to discharge. I have continued to not let my personal
beliefs enter into the discussion with the patient at any point while also getting the information required for the documentation.

Regarding this Professional Behavior, I would like to improve in the following ways: I have not had to handle any interpersonal skills with a difficult patient
as the situations have not arrived yet. As well as above in the communication section I still need to work on improving my utilization and modification of
my communication in verbal and non-verbal forms of communication like visual cues to meet the needs of different patients mainly with cognitive deficits.

Examples of behaviors to support my self assessment: I have been early to work everyday and have kept a clean environment for my coworkers and for
the patients. I have admitted when I have needed help or when I was not sure about the information to improve the safety of the patients. I have followed
through with the tasks that I have been asked to complete or to a point of completion in which help was required.

Regarding this Professional Behavior, I would like to improve in the following ways: I would like to make a safer environment for the patients by being
more prepared for the interventions, materials, restrictions, and obstacles that my be in place.

Examples of behaviors to support my self assessment: In the clinic there has been more collaboration with team members like the PTAs, OTs, and SLP
for patients that need multiple services at the same time as physical therapy. Throughout the day I have needed minor prompting to complete tasks

mainly if there was a plan for the day that I do not know much about and I have not had open access to the computer system to type up the notes
formally when I can as it has been more so when I am allowed access. Although I have also clarified responsibility of tasks mainly in the EMR for the
day.

Regarding this Professional Behavior, I would like to improve in the following ways: In the clinic I need to do more collaboration with team members like
the PTAs, OTs, and SLP for patients that need multiple services as there has not been that much overlap, but I would like to complete some further
research on the roles of the various professional as if I need to refer an individual I may with a better understanding to their scope of practice.

licensure by performing some research.

Examples of behaviors to support my self assessment: Since the midterm I have maintained the professional image through that use of the professional
dress code set by the clinic, maintaining APTA code of ethics, and I have respected those from different cultures, religious backgrounds, and beliefs both
as coworkers and as patients in verbal and nonverbal ways. I have also attended a clinic in-service along with additional practice of manual techniques
with CI, other PTs, and PTAs. I would like to have a better understanding of the regulations of the licensure by performing some research. I have done
some research on the main components of the regulations of licensure in the state of Michigan to gain a better understanding along with the scope of
practice.

Regarding this Professional Behavior, I would like to improve in the following ways: I would like to do some more research on my own about future
professional collaborations or meetings to be more involved in the profession to promote leadership within the scope of physical therapists.

been defensive towards the feedback, but more so just frustrated with myself that I am making these mistakes in front of the patients.

Examples of behaviors to support my self assessment: : Since the midterm I have incorporated the constructive feedback to make adjustments to my
skills in the evaluation and treatment sessions. I have continued since the midterm to not be defensive towards the feedback or letting the self-frustration
show in the moment or in private areas of the clinic as I have accepted the constructive feedback of the particular situation and attempted to follow

through with the correction with each patient even with the technique of the skill/exercise that I am teaching them with few/less frequent corrections that
need to be made.

increased levels of confidence while I am speaking and formulating a plan.

the notes before each session to better prepare myself. I have been asking for help when I need it for if I have a simple question.

speed up productivity and I would like to look out more during the unscheduled times for learning opportunities without being told to do so.

Examples of behaviors to support my self assessment: I arrive early to work everyday and prepare spaces for the patients along with reviewing some of
the notes before each session to better prepare myself. I have been asking for help when I need it for if I have a simple question. During chart review I
have been able to speed up my reviewing of the important information and evidence to increase my efficiency and productivity and I have been looking
out more during the unscheduled times for learning opportunities with occasional direction whom I should follow if I am allowed.

Regarding this Professional Behavior, I would like to improve in the following ways: I will continue to research various ways to handle referrals and how
to obtain them in the most appropriate and timely manner, I would also like to share more self-learned knowledge from research performed on my own
time

Examples of behaviors to support my self assessment: : Since the midterm I have stayed professional with the patients with various conditions, what has
been said by the patient or what action the patient has done. I have taken breaks when needed and have continued to perform stretches when I can to
relieve stress. I started within the last few weeks yoga at home and planning an appropriate schedule to allow proper sleep. Within the clinic I have
worked on containing my stress to shown less on my face or with my body position.

Regarding this Professional Behavior, I would like to improve in the following ways: I would like to continue to ease my stress with research and having a
plan to deal with various stressful conditions within the clinic when dealing with a particular patient or a medical emergency. I would like to incorporate
more time to exercise at home to decrease levels for stress while also dealing with my personal life.

Examples of behaviors to support my self assessment: I have done some research in common pathologies in the clinic, but still need to complete more. I
have been building my skills with each patient to improve with the correction being made.

Examples of behaviors to support my self assessment: Since the mid term I have been able to continue research on my own time with additional
supplemental information and research/studies on moderate to more complex cases. On some down times in the clinic there has been practicing of
manual therapy on various coworkers to test out the interventions and how to best use them with an applicable patient.

Regarding this Professional Behavior, I would like to improve in the following ways: I need to be more self-assessing in the moment with fewer
corrections that need to be made in the future as to the proper technique. Recently to learning has been more randomized based on the current patients,
but in the future I would like to be more in depth with moderate to high complexity cases.

Based on my MIDTERM self-assessment of my Professional Behaviors and the areas I have identified for improvement, I am setting the
following goals (list at least 2):

I want to perform my tasks in a timely manner as to not increase stress in the work environment and to improve my proficiency when I am working
with patients.
I want to improve my critical thinking and differential diagnosis skills both in evaluations and treatment sessions to be more accurate and effective as
a SPT.

To accomplish these goals, I will take the following specific actions:

I will set out time at the end of my day to study my past coursework, exercises, techniques, and research common pathologies to have a better
understanding of what I should with a patient with a particular presentation. At the clinic I will work on not spending unnecessary time that it should
normally take to complete the documentation and to make that corrections on the mistakes I have made in the past.

Based on my FINAL self-assessment of my Professional Behaviors and the areas I have identified for improvement, I am setting the following
goals (list at least 2):
I want to improve my efficiency and critical thinking skills used throughout the evaluation process to better streamline my differential diagnosis which
can by followed up with an appropriate intervention and plan of care.
I want to improve my communication and interpersonal skills with both patients and coworkers to be more effective with my communications of what
is required of them with verbal and non-verbal actions, mainly with patients with cognitive deficits that has had a stroke.

To accomplish these goals, I will take the following specific actions:

I continue to set out additional time throughout the week or weekend based on my class schedule to study coursework, evidence-based research,
evidence-based exercises/techniques, and evidence-based interventions and which pathologies or patient presentations in which these would be most
effective or indicated. In the classroom, I will set out additional time throughout the week or on the weekends based on my class schedule practice
manual therapy with my fellow classmates to gain practical constructive feedback in the moment.
