Quezon City, to be my true and lawful Attorney-in-fact for me in my name, place and steads, to do and
perform any and all acts, deeds and things, to wit”.

FORCES OF THE PHILIPPINES, update of records and Identification Card at AFP Office of The
Adjutant General (OTAG), Camp Aguinaldo, Quezon City where my signature/presence is
required, regarding my monthly pension.

2. To transact any legal matters with the said Department, to sign any all documents, papers for
said transaction.

3. To do and perform any and all business transactions, acts and deeds necessary for the
faithful execution of the foregoing authority.
