I, Florencia M. Antonio do hereby permit my son/daughter Elisha Marie M. Antonio of legal

age, and a student of Taguig City University-College of Education taking up Bachelor of
Secondary Education Major in Mathematics/ 4th Year / A2019 undergo Teaching Internship
which is one of the course requirements of the university.
I agree that the Taguig City University and the host training establishment/partner school
where my son/daughter has to undertake training is not, in any way untoward incident/injury
and health issues such as Covid-19 that may arise while undergoing training.
I further waive my rights as for any damages and liabilities and not hold the university and
the host training establishment/partner school accountable whatsoever.
