proprietor, with business address at Blk. 3 Lot 15 Tarnate
St., Talanay-B, Brgy. Batasan Hills, Quezon City;

3. That we service, supply, maintenance and repair-all types of

air conditioning units

4. That the said Business has no shop, and the repair/services is
