Dear Dr,

Re: MS Betty Johnson, 81 year old.

I am writing to refer MS Betty Johnson for her follow up care, who underwent a right total knee
replacement surgery.

On 25/03/2015, MS Johnson underwent a surgery without any complications. Although her post-
operative recovery was successful but her HB, levels were low so a blood transfusion was ordered
accordingly. Please note warfarin was ceased for 5 days prior to surgery and clexane was prescribed as a
prophylactic anticoagulant that was stopped later. In addition, she was on morphine, oxycodone, and
paracetamol for pain management. Since, the operation her wound have healed and sutures have been
removed. Thereafter, she was admitted to rehab center for further assistance and physiotherapy.

On her discharge today, no abnormalities were noted in her reports hence she was commenced on
warfarin, feratab, paracetamol and oxycodone. In addition to that, a home nursing assistance has been
recruited for her who will assist and take care of her as she is still on stick/ frame for mobility.
Furthermore, a rehab appointment is booked for her in two weeks.

In view of the above, I would be highly appreciated if you could repeat a FBE and INR test again.

Should there be any queries, kindly do not hesitate to contact me.

Yours sincerely

Dr Avinav
