WHEREAS, the members of the association have determined it to be in the best interest of the
association to open an account with Cooperative Bank of Cotabato, be it:

RESOLVED, that the association do open an account with Cooperative Bank of Cotabato with
registered address at Mlang, Cotabato.
FURTHER RESOLVED, that the following be and are hereby authorized to sign, execute, and
submit all documents in connection with opening of the account with the above said branch
including verification of signatures of authorized signatories as may be required by the
COOPERATIVE BANK OF COTABATO.
