Client Initial: Ms. S.C. Date: September 23, 2011 Time: 10am PROCESS RECORDING Introduction: I was looking for my patient; I called her by her name. At first I was hardly looking for her, she is deeply asleep. She heard her name and stood up and walked to reach me out. She grabbed the nameplate I provided for her. Patient is oriented towards date, time, place and people. Overall appearance is neat with smile on her face as I approached her. She requested for a minute to fix herself. She combs her long hair with brown highlights, and tied it. Reminded me of the slipper she wants. What nurse said or did Open-ended: Kamusta ang tulog mo? Focusing/Openended:Ano iyon? Anong nakita mo kagabi? Pwede mo bang i-share sa kin? Focusing: Ano tinitignan mo dyan sa ibaba? Open-ended: Ahh, ano palagi mong ginagawa dyan? Focusing: ah, bakit naman? Edi nakalataas yan palagi, saying wala kang slipper. Empathy: Focusing: eh ano naman yung nasa kamay mo? What client said or did Nice. And when in public naman, Im can still manage to be professional or may time na take agad ng gamot. Im taking mood stabilizers, then lifetime kong itatake yun. Then ngayon same pa rin, mood stabilizers in the morning, afternoon and evening. Patients relationship with her family is very ok. She is treated by her stepfather as if she is her real daughter. Patient denies traumatic experiences with her stepfather or being abused. Patient did not deny of her condition. She knows her condition very well. She coped with it effectively.

May mga symptoms ka pa bang nararamdaman or napapansin? What did you actually felt when you found out of your condition.

She is taking mood stabilizers and knows what theses are for. She still working until such time her business started to fail. Patient is highly compliant to meds prescribed by her physician.

Feels happy because after 4 days in the acutely ill area she was sent down and the way she treats by other patients are very different. Patient expresses worries about her usual ECT treatment. She refused the second time around because of her idea that ECT causes the brain to shrink. Thus, it will result to loss of knowledge. She is afraid of it. Patient is intelligent and knows enough more than others. She finds reading as a good habit to gain more knowledge.
