REHAB SERVICES REFERRAL LETTER

Your doctor has referred you for therapy treatment. If you are interested in scheduling
an appointment, please call the Department of Rehabilitation Services. You may call
Monday through Friday between the hours of 8:00AM through 4:30PM.

Some important information about your appointment:

 •
Wait time will vary but many people can expect to wait approximately 2 months
to be seen by the therapist.
•
The address of your appointment is listed at the top of this letter.
•
We are located on the first floor of the main hospital. Enter in through the main
entrance and before you reach the information desk turn left and go down the
hall. We are all the way down the hall to the left side.

On the day of your appointment:

•
Please arrive on time since we are by appointment only.
•
We allow a maximum of 15 minutes to be late ONLY, since we have patients
scheduled after you.
•
If you have not checked in by 15 minutes of your appointment start time, we may
cancel or give the appointment away. If you will be late, please call our office to
see if we are still able to see you or need to reschedule your appointment.

PLEASE NOTE:
Please contact us right away.  You only have 30 days from the date on this letter to
respond and schedule the appointment. If we do not hear from you within this time, the
referral will be closed out and a new order will be required from your physician in order
to receive therapy services.

una cita, por favor comuniquese con el departamento de Servicio de Rehabilitacion.
Usted puede llamar de lunes a viernes entre las horas de 8:00AM a 4:30PM.

Información importante sobre su cita:

El tiempo de espera variará, pero muchas personas pueden esperar
aproximadamente 2 meses para ser atendidas por el terapeuta.

Estamos localizados en el primer piso del hospital principal. Ingrese a través de
la entrada principal y antes de llegar al mostrador de información, gire a la
izquierda. Estamos al final del pasillo a mano izquierda.

El día de su cita:

Llegue a tiempo a su cita como tenemos citas después de usted.
•
Permitimos un máximo de 15 minutos llegar tarde ÚNICAMENTE, ya que
tenemos pacientes programados después de usted. Si va a llegar tarde, llame a
nuestra oficina para notificar a nuestro personal.

Si no se registra dentro de los 15 minutos de la hora inicial de su cita, podremos
cancelar su cita del dia. Si va a llegar tarde, llame a nuestra oficina para ver si
aún podemos atenderlo o si necesitamos reprogramar su cita.

TOME EN CUENTA:
Por favor contáctenos de inmediato. De que a partir de la fecha que aparece en esta
carta, tiene solo 30 dias para llamar y hacer su cita. Si no hay repuesta de usted entre
este tiempo, la referencia cera cancelada y una nueva orden sera requerida de su
doctor para recibir servicios de terapia.
