Operation Outdoors

Free ASYMCA Family Camp at Camp Surf September 30 - October 2


Camp Arrival and Departure Families should arrive at Camp Surf on Friday October 1st between 5:30 p.m. and 6:30 p.m. for check in. Sunday October 3rd, after a jam packed weekend, families will move out of their cabins and then enjoy a farewell Sunday lunch. Families should expect to depart camp by 1 p.m. Directions to Camp YMCA Camp Surf 560 Silver Strand Blvd. Imperial Beach, CA 91932 Take I-5 South to exit 5A for CA-75/Palm Ave Take right at CA-75/Palm Ave (signs for Imperial Beach) Turn left at Palm Ave Turn right at Silver Stand Blvd Overnight Accommodations Accommodations are assigned by family and are a combination of shared openair cabins and platform tents. Cabin assignments will be provided upon check in. Families should park in the general camp parking area. Mealtimes Standard meal times are Breakfast at 8:00 a.m., Lunch at 12:00 p.m., and Dinner at 5:30 p.m. The first meal served at this family camp is Saturday breakfast be sure to eat dinner before arriving at camp. Remember to arrive to meals on time to ensure plenty of hot and wholesome food! If you bring food to camp, store it in a sealed container to avoid animal or insects in your cabin, or luggage! What to Bring Sleeping bags Short Sleve/T-shirts Warm Shirts Sweatshirt Bathing Suit Long Pants and shorts Socks Sneakers Sandals Towel & Toiletries Flashlight Rain Jacket Hat & Sunscreen Camera Optional Equipment Wetsuit Rashguard Bodyboard *Pack n play (encouraged for families with infants) *Weather conditions are unpredictable be prepared.

YMCA Policies
Tobacco, Alcohol and Illegal Drugs As a YMCA Camp, policy does not allow smoking, alcoholic beverages or illegal drugs in camp. If you need to bring medication please make sure that it is locked and not accessible to children. Pets Pets of any kind (with the exception of guide dogs) are not permitted at camp. Cabin Care Please help maintain YMCA cabins to ensure future family opportunities. Intentional damage or vandalism of buildings may result in a repair charge and will result in removal of Armed Services YMCA program and event privileges.

Location_______________YMCA Camp Surf Imperial Beach___________________________________________ I, the undersigned parent/person having legal custody/guardianship of the above said minor, give permission for the minor to participate in the San Diego Armed Services YMCA program described above. I grant full permission for my child and/or myself to be photographed by the San Diego Armed Services YMCA staff for any legitimate purpose without payment or compensation. The minor is physically able and mentally prepared to participate in all activities as described in the announcement for the program. I hereby voluntarily and knowingly assume all risks and dangers inherent and incidental to the activities of the program. I will not hold the San Diego Armed Service YMCA liable for any injuries incurred during the program or while my child(ren) is/are in transit to and from the program whether caused by equipment or the act or omissions of others excepting damage or injury solely caused by the willful misconduct or negligence of the San Diego Armed Services YMCA, or its employees, volunteers, or agents. I do hereby authorize the San Diego Armed Services YMCA as agent for the undersigned, to consent with respect to the minors, to any x-ray examination, anesthetic, medical, dental, or surgical diagnosis or treatment, and hospital care which is deemed advisable by, and is to be rendered under general or special supervision of, any physician and surgeon licensed under the provisions of the California Medical Practice Act on the medical staff of any hospital, whether such diagnosis or treatment is rendered at the office of the physician or at the hospital. I understand that the San Diego Armed Services YMCA is not responsible for costs incurred for medical care. If I participate in the program, whether as coach, instructor, aide, spectator, or participant, I presently waive as to the San Diego Armed Services YMCA and staff, officers and directors thereof, any claim presently known or unknown for damage to property or personal injury whether caused by equipment or the acts or omissions of others including San Diego Armed Services YMCA personnel.

The San Diego Armed Services YMCA is pleased to invite your family to the Operation Outdoors Family Camp. To confirm your familys participation, this registration form and a refundable twenty dollar deposit (cash/or check only) must be submitted to the Armed Services YMCA at 3293 Santo Rd, San Diego, CA, 92124 before September 29th. Each refundable deposit will be available for return to parent signing below upon completion of the program. Should the family fail to attend the program, the deposit will be considered a donation to the non profit Armed Services YMCA.
