Subject Incident Reporting
Dear Madam, It is to inform you that on 15 Sep 2021 at 2 PM, my booked patient (Mrs. Maryam, PG- first visit on 9 Sep 2021) came for an antenatal checkup to WATIM General Hospital. She was at 41+ weeks of gestation. After checkup and ultrasound for fetal wellbeing, I called Mrs Iffat (duty nurse) to do a CTG of the patient. After CTG, the patient was upset. On inquiring, she told me that the staff nurse had told them that doctors would go for a cesarean section, so it’s better to get it done immediately. The patient was very upset on hearing this, as they had already left two private hospitals, just for this reason that they were in favour of trial for normal delivery. I counselled the patient and assured them of normal trial as there was no indication for emergency Cesarean Section. On inquiry from the staff nurse, she told me that she did not want to do any extra work and wanted to avoid night duty that's why she tried to misguide and tried to convince the patient for an immediate cesarean section. AMS was informed, she admitted all this in front of AMS as well. The next day at 2:00 PM patient came with labour pains and was admitted for the trail of normal delivery, but an emergency C-section was performed at 5:00 PM due to fetal distress. It is therefore requested that appropriate disciplinary action be taken against Mrs Iffat staff nurse for misguiding the patient and bringing a bad reputation to the hospital. It is also important to highlight that she has a previous history of misconduct/misbehaving, unpunctual, lack of responsibility and disobedient attitude. At the same time, nursing staff has no right to diagnose or advise any treatment contradicting to the patient’s consultant.
