Registration Fee: $65 (inclusive a set of Jersey and a pair of Socks)

Training Fee: $100- 1 month for 4 sessions

*Kindly make payment on the same day of Registration*

COMMITMENT

1 Season ( 10 months )
*There will be no make up class or refund for absentees
* In the event if the sessions is cancelled by the Coach there will be a make up sessions

SCHEDULE/MISSED LESSONS:

- Students must be punctual for all lessons. No additional time given for late-comers. No
additional/make up class for Absentees.
- The academy needs to receive notifications of an anticipated absence at least 24
hours in advanced

In the event that the lesson is cancelled by the coach, the coach will arrange to do a
replacement lesson

WITHDRAWAL / TERMINATION OF NOTICE:

- Any withdrawal/termination the academy needs to receive a notification of student(s) at least 7

days in advanced.

- No refunds of payment for any withdrawal/termination/absentees

PARENTS/GUARDIAN AGREEMENT
As a parent/legal guardian of the applicant/student, I hereby give permission for my child to
join/participate in the Bayview Football Academy and/or programme agree to comply with all the
programme regulations. I hereby exonerate the Academy from any loss, and all liability from injuries
during my child’s participation in the football academy. I do hereby state that my child is medically
qualified to attend Bayview Football Academy. I do hereby authorize Bayview Football Academy to act
for me according to their best judgement in any emergency requiring medical attention. I authorize
the use of photo/video taken of my child during this programme for Bayview Football Academy
materials.
