AFFIDAVIT OF LOSS
I, DANILO CALATRABA VIDA, married, of legal age, Filipino, with
address at 13 Zamora St., Barangay IV (Pob), Alfonso Cavite after having been
sworn to in accordance with law, hereby depose and state, that:

1. I was apprehended by an LTO enforcer for “no helmet”, he then issued me a

Temporary Operator Permit (TOP);

2. However, upon checking my files, I found out just recently that the said
Temporary Operator Permit (TOP) was misplaced and cannot be found

3. I exerted earnest efforts to locate the said TOP but the same could no longer
be found and should now be considered lost and beyond recovery for all
intents and purposes;

4. I am executing this affidavit to attest to the veracity of my statements and to

support the same in completing my requirements for reacquiring my Driver’s
License form the Land Transportation Office and for whatever legal purpose
this may serve

IN TRUTH WHEREOF, I have hereunto affixed my signature this 24 th day of
