Team: Health and Wellness Team Reports to: Executive Director FLSA Status: Exempt from overtime Salary Range: $14.98-$19.15 per hour Schedule: regular part time, 25 hrs per week Last Revised: September, 2011

The Community Wellness/Diabetes Program Coordinator offers healthy lifestyle options through use of nutritional consultants and support groups, various physical and mental activities and in coordinating with Native Village of Eyak and other agency programs. This position establishes methods and procedures for attaining specific goals and objectives, and receives guidance in terms of broad goals. Only the final results of work are typically reviewed. Responsibilities/Duties Prepare and facilitate weekly Nutrition Education/Healthy Lifestyle Support Group. Conduct research to provide accurate and appropriate health information for community members. Educate individual patients toward healthy lifestyle change by referral from health care providers. Be familiar and facilitate a good working relationship with other agencies in town and around the State. Coordinate with contract registered dietician to encourage good nutritional lifestyles. Create and prepare public service announcements and advertisements for local TV scanner channel, radio, and newspaper. Provide community physical activity opportunities for all ages. Provide, clean and maintain physical activity equipment program at Masonic for community use, acquire new equipment as needed. Arrangement and facilitation of guest speakers for Health/Wellness Information Presentations ( previous topics covered by guest speakers; Naturopathic Medicine, Reiki, General Nutrition, and Homeopathy) Work closely other NVE Programs to provide beneficial and appropriate nutrition, health and wellness information and support. Work with various programs to support the Sobriety Celebration. Maintain healthy lifestyle libraries for use by the community, Liaison with Cordova Public Schools to make presentations and/or classes on health lifestyles. Prepare and submit any necessary reports. Experience Requirement: One year experience in nutrition therapies and lifestyle modification modalities preferred Education Requirements: Highschool diploma or equivalent, post secondary education in nutrition, healthy living preferred.

Competencies (Knowledge, Skills and Abilities) Must be able to work with Alaska Native Tribal members and other diverse cultures and modify programs based on social and cultural norms. Must have excellent oral and written communication skills. Must have the urge to help people regardless of income or life choices. Must have knowledge of medical systems Skilled at listening and allowing people to choose their own methods of care Excellent computer skills Ability to act professionally and respectfully with clients, colleagues and others Performance standard Must be physically fit in order to perform exercise classes and other physical activities. Must be able to move quickly and sustain physical activity for an hour or more. Regularly bend and/or crouch in order to complete tasks. Typically use sight and touch to complete job tasks accurately. Typically lifts 15 lbs to coordinate work Regularly manipulate electronic data to gather, input and otherwise coordinate work.
Regularly use mental, oral and written methods to complete work.

Environmental Factors Majority of work will be completed indoors; however outdoor activities will be completed as necessary and may involve exposure to inclement weather, swift water and dangerous wildlife.
