WAIVER, RELEASE, AND QUITCLAIM

including my agents, executors, administrators, and successors-in-interest, forever
remise, waive, release, and forever discharge FORTRUST SECURITY AGENCY
CORPORATION represented herein by: ARVIN JEFF BERNARDO (the
“Corporation”), a private corporation duly organized and existing under
Philippine laws with principal address at No. 15, Brgy. Sto. Tomas, Magarao,
Camarines Sur, any claim or right by reason of my past employment with the
Corporation or any matter, cause or thing whatsoever arising out in connection
with the same.

I hereby affirm that I have executed this instrument out of my own free will
without force, intimidation, or violence upon my person, and that I have no more
claim or demand against the Corporation, in connection with the above-
mentioned employment.

I further warrant and represent that I will not institute and hereby forever
waive any action, whether legal, administrative or of any other nature and kind
whatsoever which I had, now have or hereafter may have against the Corporation,
arising out of, or in connection with the above-mentioned employment or
assignment.

Finally, I declare herein that my receipt of the aforesaid amount was

voluntarily as well as the waiver of all claims which I might have against the
Corporation, without force, intimidation, or violence, and the content of which
were fully explained to me in the dialect that I understood.
