Chapter I

INTRODUCTION

1.1 Background of the Study

Tuberculosis (TB) is a serious, potentially life-threatening infectious disease

caused by a bacteria called Mycobacterium tuberculosis. It is a disease that primarily

affects the lungs, though it can also affect other organs such as the brain, spine, and

kidneys (Cleveland Clinic, 2022). Additionally, it is an air-borne disease and can be

transmitted easily, thus making it highly infectious. Because of its contagious nature,

TB remains a significant issue, with around 1 million active cases recorded in the

Philippines as of 2019 (World Health Organization, 2019).

Monitoring the treatment status and occasional progress of a patient is

important when combatting different diseases, including TB. Patients receive

treatment once diagnosed with the disease and are isolated in order to prevent

infection. This treatment usually lasts for at least 6 months, with a monthly check-up

with their doctor (Centers for Disease Control and Prevention, 2021). Despite the

need to monitor a patient’s progress, there are times when the patient does not

regularly update their doctor, drink their medicine, and even fail to show up for their

monitoring. Because of this, their treatment for TB is not as efficient as expected.

Machine learning is a rapidly developing branch of Artificial Intelligence (AI). It

is a scope of computational science that focuses on the analysis and interpretation of

patterns and structures in the data for the AI to learn, make predictions, and many

more (NetApp, n.d.). This branch of AI can be found in search engines, email filters,

and even voice recognition (The Royal Society, n.d.). It can also be used for the

development of monitoring and filing systems.

The purpose of this study is to develop an application that will help monitor TB

patients from their homes and create a database that will show the status of certain

patients. The study aims to identify whether the application will be accurate,

efficient, and easy to use; whether it will display necessary information; and whether

the application will help with easy filing and patient profiling.

1.2 Significance of the Study

The utilization of Machine Learning in the development of an application

whether it’s able to properly determine and monitor TB patients. 

This study will benefit various sectors and people are as follows: 

Department of Health (DOH)

This study will benefit this department as our application is able to help keep

track, monitor, and profile TB patients and help collect data much easier and

convenient.

Philippine Tuberculosis Society, Inc 

This study will benefit this organization in various ways as our application is

able to help monitor and keep track of TB patients as well as providing further

significant information about the disease itself. 

Hospitals/Medical Centers 

This study will benefit these places as it’ll give them a much easier time to help

keep track of TB patients encountered everyday as well as to monitor them of

proper medicine intake and regular check-ups. 

Pulmonologists 

This study will benefit these medical professionals as our application will be able

to help record data and keep track of TB patients inputted by them and through

the doctors. 

Community

This study will benefit the community for it will be a much easier and

convenient way to identify TB patients and a way for them to keep track and

monitor their own current status and well-being. 

1.3 Statement of the Problem

Tuberculosis (TB) is a serious issue in the Philippines. As a way to combat

the disease, it is important to provide and monitor the treatment of a patient.

However, monitoring a patient can be difficult, especially when they are diagnosed

with an easily transmitted disease. In order to ease the situation, an application for

monitoring patients and filing that is accessible to both the patient and doctor will be

developed.

In order to ensure the accurate and efficient performance of the

application, the study aims to answer the following questions: 

1. Will the application be accurate, efficient, and easy to use?

2. Does the application display and label the necessary information, namely the

progress of the patient, the information the user will input and whether their

treatment is progressing smoothly?

3. Will the application properly organize files of TB patients and make it easily

accessible to their doctors?

1.4 Research Hypothesis

The following are the hypotheses formed:

1. The application will not be accurate, efficient, and easy to use.

2. The application does not display and label the necessary information, namely

the progress of the patient, the information the user will input and whether

their treatment is progressing smoothly.

1.5 Conceptual Framework

The study aims to develop an application that will be able to monitor the

tuberculosis (TB) patients easier through machine learning and to determine the

accuracy of the system created.

The figure above shows the relationship between the independent and

dependent variables of this study. The algorithm used to file/monitor TB patients,

together with the programming is the study’s independent variable (IV). Meanwhile,

the application’s feasibility and effectiveness to file the patient’s conditions is the

dependent variable (DV).

1.6 Scope and Limitations

This study will focus on developing a machine learning-based application for

filing TB patients by programming the application. This study aims to implement an

application that will be able to locate and identify patients who have completed or

uncompleted their medications that would improve the efficiency of filing in hospitals

and health centers. The study does not aim to produce a physical machine nor make

it commercial. The intended program and application will not be made publicly

available and will serve as a temporary program, open to future modifications

dependent on the successful validation of the study. 

The programming aspect of the project will be carried out using the Python

programming language. To evaluate the feasibility of the application, an observation

analysis will be conducted through changing the time to assess whether the

application does work.

The independent variable of the study, the set of instructions to be utilized to

analyze the patient’s data and provide valuable insights, enhancing the

effectiveness of the system. Several algorithms are available for machine

learning.

Chapter II

REVIEW OF RELATED LITERATURE

2.1 Conceptual Literature

Tuberculosis

Tuberculosis (TB) is a potentially fatal illness that mostly affects the lungs.

Mycobacterium tuberculosis is the kind of bacteria that causes tuberculosis (TB)

(Mayo Clinic, 2023). Tuberculosis spreads quickly in places where people

congregate or live in close quarters. The general symptoms of Tuberculosis

include feelings of sickness or weakness, weight loss, fever, night sweats,

coughing up blood or mucus, chest pain, pain with breathing or coughing, and

fever. TB sickness symptoms vary based on the part of the body affected

(Centers for Disease Control and Prevention, 2021). The most often used therapy

for active TB Isoniazid is INH in combination with three other drugs—rifampin,

pyrazinamide, and ethambutol. Although you may feel better within a few weeks

of taking the medications, treating tuberculosis takes significantly longer than

treating other bacterial diseases (American Lung Association, n.d.). Current TB

treatment monitoring recommendations rely on sputum and culture conversion,

which have low sensitivity and long turnaround times, pose a biohazard risk, and

are prone to contamination, undermining their utility as clinical treatment

monitoring tools and for drug development (American Society for Microbiology,

2022).

Machine Learning

Machine learning is a continuously evolving branch of Artificial Intelligence

wherein the system makes use of data and algorithms to learn on its own and do

certain tasks (IBM, n.d.). In contrast to other systems, machine learning

algorithms utilize computational methods to learn from data, instead of

depending on an ingrained equation (MathWorks, n.d.). It is in the field of

computational science which involves the analysis and interpretation of data

patterns to support learning, reasoning, and decision-making, without the

involvement of humans (NetApp, n.d.).

2.2 Related Studies

FOREIGN STUDIES

A study titled "What We Know About Tuberculosis Transmission: An

Overview," conducted by Churchyard et al. aims to provide a comprehensive

understanding of tuberculosis transmission and recommend targeted interventions.

The study presents a simplified cascade model of transmission, highlighting the

sequential events from the source case generating infectious particles to susceptible

individuals becoming infected. By targeting each stage of the transmission cascade,

interventions can effectively disrupt tuberculosis spread, leading to a decline in

incidence and mortality rates. This study lays the groundwork for future articles

exploring specific aspects of tuberculosis transmission, offering valuable insights for

combating this global health issue.

A study titled "Drivers of Tuberculosis Transmission," by Mathema et al. aims

to explore the complex factors influencing the transmission of tuberculosis (TB)

within communities. The researchers discuss the challenges in measuring TB

transmission and highlight various approaches used to estimate the probability of

recent transmission and the fraction of TB cases attributed to recent transmission in

populations. These approaches include analyzing tuberculosis case notifications in

young children, trends in tuberculin skin tests and interferon γ–release assays, and

the use of M. tuberculosis whole-genome sequencing to track transmission patterns.

The study emphasizes the significance of factors such as the prevalence of TB, living

and working conditions, and the effectiveness of healthcare and public health

systems in identifying and treating infectious cases. By providing an overview of

these factors and measurement tools, the study aims to bridge knowledge gaps and

contribute to the understanding of TB transmission dynamics.

Wang et al. conducted a research study with the title of "Using Electronic

Medication Monitoring to Guide Differential Management of Tuberculosis Patients at

the Community Level in China," the study aimed to evaluate the acceptance and

effectiveness of an electronic medication monitor (EMM) for supporting TB patient

adherence to treatment in a community setting. The study was conducted in

Zhenjiang City, Jiangsu Province, where universal implementation of directly

observed therapy (DOT) was not feasible. Patients were given the option to use the

EMM or receive DOT. The EMM stored a month's supply of anti-TB drugs, and patient

adherence was monitored monthly. Patients with poor adherence were switched to

DOT. The study included 231 TB patients, with 154 completing the treatment course

using the EMM. The median adherence rate was 99.3%. Healthcare workers found

the EMM acceptable, although it moderately increased their workload. Through the

EMM program, the workload of community physicians significantly reduced by

reducing patient visits. The study highlights the potential of EMM for differential TB

patient management at the community level in China, but further operational

research is needed before scaling up the technology.

A study conducted by Wang et al. entitled "EMM Implementation for TB

Patients in China" assessed the use of an electronic medication monitor (EMM) for

self-administered TB treatment. Out of 2,227 individuals, 1,810 (81%) were eligible

for EMM, and 1,314 (73%) used it during their treatment. Based on EMM data, 134

patients (10%) required a shift to directly observed therapy (DOT). Factors

associated with not using EMM within the first month included age, employment

status, tuberculosis pleurisy, and previous treatment. The study emphasized the

need to ensure adherence and provide clear guidelines for managing missing EMM

data. EMM implementation holds promise for enhancing TB treatment and control in

China.

A study titled "Evaluation of Mobile Application for TB Management in Tianjin,"

by Li et al. assessed the effectiveness of a mobile app in improving adherence and

patient outcomes. Data from the app and TB Information Management System were

analyzed using logistic regression. App users had more regular check-ups, and

33.28% showed regular drug intake. The overall drug intake rate was 84.84%. Poor

adherence was linked to tuberculosis pleurisy and prior treatment, while residents

had better adherence. The study highlights the app's positive impact but suggests

additional measures for better medication compliance.

A study entitled “An artificial intelligence approach to monitor student performance

and devise preventive measures” was conducted by Khan et al. in 2021. The study

aims to monitor students’ academic progress with machine learning, as well as

inform their respective instructors to help improve the students’ grades at an early

stage. The researchers created a system by utilizing different machine-learning

algorithms. The results showed that the Decision Tree algorithm worked the best

compared to other algorithms. It was concluded that machine learning can indeed

monitor student performance and suggest preventive measures to assist the

students with their struggles.

LOCAL STUDIES

A study entitled "The social determinants of Tuberculosis in the Philippines"

was conducted by Flores et. al in 2022. The study states in the Philippines, where

nearly 10 million people reside in urban slums, social determinants are also major

drivers of tuberculosis endemicity. The local 2019 TB Program Review showed how

vulnerable populations—the urban poor, people living with HIV, and people

deprived of liberty—are more susceptible to infection and worse treatment

outcomes. The study contributes to the importance of the study because of the

frequent victims of Tuberculosis.

Calderon et al. (2022) conducted a study titled "Innovating tuberculosis

prevention to achieve universal health coverage in the Philippines." The goal of the

study was to make a significant contribution to tuberculosis (TB) elimination by

advocating for the integration of innovative approaches such as tele-contact

investigation (TCI), mathematical modeling, and participatory governance in TB

preventive treatment (TPT). The findings of this study emphasize the importance

of adapting innovative strategies to address TB elimination and improve TB

preventive treatment, proving the importance, timeliness, and valuable insights for

our current research.

A study entitled "Barriers to tuberculosis care in the Philippines" was

conducted by Dychiao et.al in 2022.  The paper reported a discrepancy between

expected and reported tuberculosis cases during the COVID-19 pandemic. This

study aimed to examine the barriers to tuberculosis care in the Philippines amidst

the COVID-19 pandemic. Using a combination of quantitative and qualitative

research methods, the researchers investigated factors such as public perceptions,

healthcare access, and the impact of the decentralized COVID-19 response on

tuberculosis surveillance and control measures. The study revealed a significant

decline in tuberculosis case notification, with a decrease of 78.7% following the

implementation of community quarantine measures. Public perceptions about

respiratory illnesses, limited healthcare access, and the decentralized COVID-19

response were identified as major hindrances to tuberculosis care. Furthermore,

the study highlighted the projected increase in tuberculosis incidence and deaths

due to the effects of COVID-19. The National Tuberculosis Program (NTP)

interventions, including video-observed treatment and digital adherence

technologies, were found to be promising, resulting in a 75% relative reduction in

loss to follow-up after 6 months of treatment. However, these interventions were

limited to only two regions and remained inaccessible to the majority of Filipinos.

The study emphasizes the need for a centralized strategy to enhance surveillance,

service delivery, and the overall reduction of the tuberculosis burden in the

Philippines. This study helps the current one as it points out the need for a system

that monitors and keeps track of TB patients in the Philippines amidst the current

barriers and problems it faces.

2.3 Synthesis

The related studies above include a variety of information that have similar and

different results or components used compared to our study. The study of

Churchyard et. al (2017) and Agyeman et al. (2017) lays the groundwork for future

articles exploring specific aspects of tuberculosis transmission, offering valuable

insights for combating this global health issue and offers general education to

healthcare professionals and the public which promotes awareness of its prevention

and management strategies. Furthermore, The study of Li et. al (2021) included data

from the app and TB Information Management System were analyzed using logistic

regression. App users had more regular check-ups, and 33.28% showed regular drug

intake. This shows the importance of a monitoring app and why the logistic

regression algorithm was used. Additionally, the study of Mathema et. al discusses

the challenges in measuring TB transmission and highlight various approaches used

to estimate the probability of recent transmission. These challenges include analyzing

tuberculosis case notifications in young children, trends in tuberculin skin tests and

interferon γ–release assays which bridge knowledge gaps and contribute to the

understanding of TB transmission dynamics. The studies of Wang and Shewade et. al

utilized electronic medication monitors (EMM) for supporting TB patient adherence to

treatment in a community setting. Healthcare workers associated with the study

found the EMM acceptable and states EMM implementation holds promise for

enhancing TB treatment and control which justifies the application of EMM in our

study.

2.4 Justification of the Study

Being one of the leading causes of diseases in the Philippines, the study will

be conducted to utilize machine learning in the creation of an application to monitor

and keep track of TB patients among medical centers and hospitals for medical

personnel and TB-affected patients to use. The preceding foreign and local studies

disclosed different and consistent findings about the lack of monitoring and

evaluation of TB patients and the need for one. The variables are with similarities to

the present study, specifically, with the use of Python as the computer program, and

data set to monitor and evaluate. The researchers wanted to compare the

differences between each study, taking into account the gaps and recommendations

of each supporting study that the researchers wish to enhance and address.

Numerous foreign and local studies including the study of Li et al, Khan et al

(2021), and Wang et al have correlations to the present study because they were

able to highlight the need to approach the innovation world in the fields of computer

programming language, mathematical modeling, tele-contact investigation. This laid

the groundwork for the creation and the groundwork for the variable of the paper.

Taking into account the similarities of the supporting studies to the present,

there are gaps and recommendations that the researchers would like to improve.

The related literature entitled “EMM Implementation for TB Patients in China”

ensured the efficiency of an electronic medication monitor, unlike the main study, it

does not have various algorithms to compare to.

Based on previous studies, results, and discussions, the researchers decided to

perform this study to create an application using machine learning to enhance cancer

the efficiency of the health center and hospital's way of profiling, monitoring, and

ultimately tracking down the progress of each patient. Using machine learning, a

method that is widely known allows software applications to become accurate at

predicting outcomes. The study is considered a cost-efficient and efficient way to

monitor and file TB patients because an application beats the traditional way of filing

patients through papers. It is a study unlike the given related literature as the study

focuses on the accuracy and precision of the application's ability to file and monitor

TB patients through machine learning.

METHODOLOGY

3.1 Research Design

The study will utilize the One-Group Posttest-Only Design as it is the most

3.2 Materials and Procedures

Acquiring the Data

The data to be used for the training and testing of the system will be

obtained from Our Lady of Guadalupe Medical Specialists Clinic in Imus, Cavite.

The hospital will provide the data on 200 patients’ weight, cough status,

presence of colds, fever, rashes, and abdominal pain.

Data Preparation

The acquired data will then undergo a preparation stage involving data

cleanup, data normalization, and variable selection. Data cleanup will fill in null

values, while null values that cannot be filled will be eliminated. This is followed

by data normalization where the data will be converted to a form that is easier

for the algorithm to understand and analyze. Afterwards, a target variable will be

selected. The target variable or the goal of the application for the study is to

identify whether the progress of the patient is progressing well.

Algorithm Selection

After the data is prepared, the machine learning algorithms that the

program will use are selected. These are the Decision Tree algorithm, k-Nearest

Neighbors algorithm, and the Artificial Neural Network Algorithm.

System Programming

After the algorithms and data set is ready to be used, the application’s

programming will be conducted. The programming will make use of the Python

programming language, using the Python 3.11 compiler. The data set and

algorithms will be inputted into the program. Additionally, through programming,

the application will also feature a filing system for medical staff.

Model Training

The ten-fold cross-validation method will be used to train the model. The

data set will be randomly divided into nine parts for the model’s training, while

one part will be for its testing. This process will be repeated ten times with

different randomly divided parts. In order to avoid bias, ten randomly selected

information of patients will be set aside for testing alone.

Model Testing and Data Evaluation

After training the model, the application will be run. The data set aside will

then be used for testing the system and the conclusion to be presented by the

program will be evaluated in the confusion matrix, either a True Positive (TP),

False Positive (FP), or False Negative (FN). This will be tested for each of the

algorithms used by the program. The results will then be computed using the

derivatives of the matrix, namely its accuracy, determined by the formulas

accuracy = (TP+TN)/(TP+TN+FP+FN), precision = (TP)/(TP+FP), recall =

(TP)/(TP+FN), and f-score = 2*(precision*recall)/(recall+precision).

Furthermore, the smooth navigation and intended use of the application will also

be tested by simply imputing the data for testing and observing whether the

correct data is presented by the application.

Application Testing

After the first testing phase of the application. It will be sent to 5 hospitals,

namely St. Dominic Medical Center, Central East Asia Medical Center, Our Lady

of Guadalupe Medical Specialists Clinic, Bacoor City Health (Salinas) – DOTS, and

Bacoor Doctors Medical Center for a test run. The application will be used to

monitor TB patients for a month. Afterwards, the mentioned hospitals will be

answering a checklist prepared by the researchers in order to assess the

performance of the application.

3.5 Statistical Treatment

The study will make use of the T-test statistical treatment in order to

determine the means or the difference between the results provided by the doctor’s

data and the results presented by the application. This will provide a t-value and p-

value, along with its level of significance, allowing the researchers to determine

whether the study’s results are significant.
