AFFIDAVIT OF KINSHIP

I, MINERVA R. CABANIG of legal age, widow, Filipino Citizen, and a resident

of Dumlog, Talisay City Cebu Philippines, after having been duly sworn to in
accordance with law do hereby depose and state:

That I am the granddaughter of Juanito De Los Reyes who died on August 2, 1972;

That my grandfather Juanito De Los Reyes has no living parent, living spouse,
living children, or living grandchildren except me;

That In view of the foregoing circumstances, I am the surviving nearest kin of my

deceased grandfather Juanito De Los Reyes;

That I execute this Affidavit for the purpose of securing copy of the death
certificate of my grandfather Juanito De Los Reyes in the Philippine Statistics
Authority (PSA);

That the above-mentioned requested document is necessary for me to file a

petition for the issuance of duplicate copy of the title registered in the name of
my grandfather Juanito De Los Reyes which I lost while in my possession

That I execute this Affidavit in order to attest to the truth of the foregoing facts
and for whatever legal purposes that this Affidavit may serve.
