DIABETES MELLITUS, ARTERLAL HYPERTENSION AND DYSLIPIDEMIA IN INDIVIDUALS
WITH AGE -RELATED CATARACTINATERTIARY CARE CENTRE IN sOUTHKERALA" and
approved the above study to be conducted by Dr. Monica Christopher , PG Student, Department of
Ophthalmology guided by Dr. Sheldon J. Goudinho, Professor & Head Department of
Ophthalmology , Dr. SMCSI MedicalCollege, Karakonam.

The Ethics Commitee reviewed the following documents.

3. Patient information sheet & Informed consent form in Malayalam, English & Tamil.

The Ethics Committee approves the study to be conducted in the present form. Any subsequent
deviation/modification in the study has to be submitted to the Ethics Committee for its
approval.

said study during the meeting.
