residence address at 88 St. Guiller Street Bayanan, Muntinlupa City, after having been
deposed in accordance with law, do hereby state that:

1. My true and full name is ROMMEL JAMES Y BRAVO and I am a resident of 88

St. Guiller Street Bayanan, Muntinlupa City, as appearing in my Commission on
Elections ID with Voter’s Identification Number 222-333-33-4 and Home
Development Mutual Fund (PAG-IBIG) ID with number 0101-0101-0101 attached
herein as Annex “1” to “1-A”;

2. I am not the same person with the name and information stated below:
