Praxis Incident Supplemental Narrative
On 01/22/2022 I, Officer Belda, along with Officer Carlson

and Officer Steele responded to 1529 Lancaster (Praxis
Rehab) in reference to a physical fight that had occurred
between two residents.
Upon arrival we were let into the building. There was a male
sitting in the lobby and he was holding his right hand. This
subject was later identified as Nicholas Zupko. Nicholas
was calm in his demeanor and advised us that he believed
his hand was broken. I observed his hand was red and
appearing to start to swell. I asked Nicholas if he needed
EMS to check him out and he declined immediate
treatment. I asked if he would like for us to transport him to
the hospital to which he agreed. It was soon discovered that
Nicholas would be arrested for Battery. I advised Nicholas
that we would need to have his hand checked out before
going to jail.
Nicholas spoke with Officer Carlson first and I spoke with

the director of Praxis. I was advised that Nicholas was no
longer welcome due to the physical altercation that
happened. He stated that he would be gathering Nicholas'
property for him.
While interacting with Nicholas and having a casual

conversation with him, Nicholas told me that he had spend
numerous years in prison and that what the other subject
had said would be an instant fight in prison. He advised he
knew what he did was wrong and it was just a culmination
of going through withdraw from substance abuse, anxiety
from all the people in close proximity to him, and other
various things that came to a head. Nicholas was
apologetic and wanted to tell the other party that he was
sorry for what happened. Due to Nicholas' injury, he was
placed in a belly belt and handcuffed in the front. The
handcuffs were checked for tightness and double locked.
Nicholas was escorted to my patrol vehicle and transported
to Bluffton Regional Medical Center.
Nicholas was very cooperative throughout our interaction

with him and appeared genuinely apologetic for his actions.
Nicholas was willing to write out a statement but was
physically unable to due to his hand. Officer Carlson
conducted an interview at a later time.
Nicholas was subsequently discharged from the hospital

with a splint on his finger. The doctor advised that his finger
was cracked and he encouraged him to seek treatment
from an orthopedic doctor. I placed Nicholas back in double
locked hand restraints with the belly belt and escorted him
to my patrol vehicle. I then transported Nicholas to the
Wells County Jail without incident. Nicholas was accepted
by jail staff.

On today's date, officers were called to Praxis, at 1529

Lancaster Street
in reference to a battery. Officer Carlson, Officer Belda and
this officer went
to the location. I observed a male subject sitting in a chair,
with other
individuals sitting at a desk and another female who began
speaking to officers.
It was learned that the male subject sitting in the chair was
the suspect in
the battery. The male suspect, later identified to be
Nicholas Zubko was making
statements as to being sorry for what he had done, as well
as making statements
as to the other person involved getting into his face.
Throughout the time I
was at the facility, it was decided that the suspect may have
a broken hand, and
the final determination was that the suspect would be
arrested, but would
receive medical attention to his hand prior to being taken to
the Wells County
Jail. Officer Belda transported the suspect to BRMC ER for
the injury to the
hand.
Officer Belda did speak with the suspect concerning his
involvement, as I
spoke with a female who identified herself as the Executive
Director of Praxis.
Due to obtaining a statement, I was able to identify her to
be Olinka Clark.
She stated that Nicholas (suspect), prior to committing the
battery, was telling
her that he was going to go off, and he was going to punch
someone. She stated
his language was vulgar and she had told him to stop the
language he was
speaking. She stated that he then went outside, where
there is a smoking area,
and got into an altercation, punching another patient at the
facility. She
stated that the victim did not fight back in any way. I
explained to Olinka
that I would need a statement from her. She asked if she
could type the
statement on a computer and give it to this officer, and I
explained to her that
it is necessary that she hand write the statement on a
Bluffton PD form. She
was not overly excited about writing the statement, making
a verbal statement at
the time, that if she knew this, she would not have gotten

involved in the
situation. She did take a statement into a back room and
completed the
statement, giving it back to this officer. In the statement,
she states that
she observed the victim, Robert Townsend with a bloody
lip, and that Robert
stated he was attacked by Nicolas Zupko, another patient at
the facility.

On the above date and time, I was notified by Dispatch

about a subject at Praxis (1529 Lancaster ST, Bluffton, IN
46714) who was actively cutting himself with glass and
refusing to give the glass up.
Upon arriving to Praxis with Officer Ford, we were let in

the East door by an employee. Officers Pitman, Daggett,
and Monce were already in the room with the subject, later
identified as Donta Glanville (DOB 11/22/1994). Donta's
wounds were only superficial, and Donta appeared to be in
an incoherent state. Donta would not respond to any
questions asked by Officers and was flailing his body
around.
After several minutes of attempting to talk to Donta, he

began to start slamming his head back on the floor in an
attempt to hurt himself. Officer Daggett then placed a
pillow under Donta's head to prevent him from further
harming himself. At this time, Donta began talking in
breaths and gesturing like he was going to spit towards
Officers. After first holding the pillow above his head to
prevent the spittle from hitting Officers, I was able to place a
surgical mask on Donta to prevent getting spit on. I then
held his head down on the floor so that he would no longer
continue to hit it.
Once medics arrived to assist Donta, Officers

attempted to sit him up so that the medics could attempt to
talk to him. A spit hood was also placed onto Donta so that
Officers and medics would not get hit with spit. Like with
the Officers, Donta refused to speak to medics and uttered
"Fuck you" to them. Due to Donta's agitated mental state,
medics elected to transport him to Bluffton Regional
Medical Center (BRMC) so that he could be evaluated.
At this point, Donta had become more agitated and

began swinging his legs towards Officers in an attempt to
kick us. It was decided to place Donta in handcuffs for his
own safety and to prevent him from hurting Officers. I
placed Donta into handcuffs which were double locked with
proper fit and tightness. Once in handcuffs, Donta
continued his aggression towards Officers and attempted to
kick Officer Monce. Donta was given several commands
to stop kicking.
Officers then decided to roll Donta over to his stomach

to prevent further kicks. As Officers were flipping Donta, he
continued to kick despite continued commands to stop.
Once Donta was on his stomach, Officer Monce attempted
to gain control of his legs. Because Donta was still actively
kicking towards Officer Monce, I placed my knee on his
lower back to pin him to the ground, using the least amount
of pressure necessary to control him. After approximately 4

or 5 seconds, Officer Monce said that he had control of

Donta's legs, so I removed my knee from his lower back
area and transitioned to holding down his right shoulder
with my hand.
Once Officers had full control of Donta, he was picked

up and placed onto the stretcher so that he could be
transported to BRMC. Donta was placed onto the stretcher
and loaded into the ambulance. Once in the ambulance, I
assisted Officer Daggett in handcuffing Donta to the
stretcher. After Donta was secured to the stretcher, Officer
Daggett rode with Wells County EMS to BRMC. Officers
Ford, Griner, and I followed to assist at BRMC.
At arrival to BRMC, Donta continued to resist any

attempts over control over him. He had unbuckled the
straps on the stretcher and continued to flail his arms and
legs as he was taken into the hospital. Officers Ford,
Daggett, Griner, and I assisted medics transfer Donta from
the stretcher to the hospital bed. Donta was handcuffed to
the hospital bed so that BRMC staff could safely perform
their medical tests and care for Donta. We assisted BRMC
staff with their treatment of Donta. Once BRMC
administered mediation to sedate Donta, they informed us
we were no longer needed to assist once Donta was placed
into soft restraints.
This concludes my involvement with this incident at this

time.

On 2/2/21 at approximately 00:47 hours myself and Officer

Jojola arrived at Praxis reference a request for a transport
of a homicidal patient. We made contact with the
complainant and employee Bradley. Bradley stated they
were requesting a patient identified as Terese Poling (DOB
11/13/80), be transported to Parkview Behavioral because
she was homicidal toward another patient there named
Brittney. Bradley stated at this point she has to be evaluated
because she circled "yes" on a piece of paper stating that
she was homicidal. Bradley stated she could continue
treatment once the evaluation was done if she had not been
held for more than 24 hours. At this point we requested to
speak with Teresa.
Myself and Officer Jojola spoke with Terese alone and

separate from staff. Terese was asked about the incident
earlier in the night. She stated it was just a
misunderstanding and she had no homicidal thoughts and
denied making any statement about that. When asked
about the paper she stated she must have circled the wrong
one by accident. The entire time speaking with Terese her
behavior was normal and calm. She had just been woken
up by employees to come speak with us. At this point, it
was apparent that Teresa did not meet CIT Criteria after
speaking with Officer Daggett over cell phone.
It was explained to Terese that Praxis staff was requiring

her to be evaluated if she wanted to continue treatment
there. Teresa stated she wanted to continue treatment
elsewhere and would leave on her own. Teresa was able to
get property from her room only. Praxis staff was unable to
get her wallet and cell phone for her due to policy. Terese
was transported by Officer Jojola to Americas Best.

down. Sgt. Baker completed the 24 Hour paperwork and I
began the transported to
Parkview Randallia. It should be noted that Pedro's
behavior began to change at
SR 1 and 350 N. I asked Pedro if he was okay and he
stated that he felt like he
was going to have a seizure. At this time I radioed for
medics and pulled over.
Medics arrived on scene and decided that they would

complete the transport to
Parkview. I followed the medics and provided the security
staff at Parkview with
the 24 hour paperwork.
This is the end of my involvement in this matter.

On April 18, 2022 Jason McDonald called 911 and

requested an officer at the
corner of Washington and Marion. Jason advised he found
a rope and wanted to
hang himself. Officer Carlson arrived and confirmed Jason
was suicidal. I
arrived and Jason was placed into a belly belt and
transported to the police
department. Jason stated he had recently left Praxis
rehabilitation and was
homeless. Jason said he told the Praxis staff he didn't want
to be there before
he was even checked in. Jason said the Praxis staff
advised him they wanted him
to check in then they would discharge him. Jason said he
believes they wanted
him to check is so they could receive payment from
Medicaid. Praxis had given
him a ride from the Indianapolis area to Bluffton but left him
homeless on the
streets of Bluffton when he asked to be moved to a different
facility.
I completed the 24 hour paperwork and Officer Daggett

transported Jason to
Parkview ER where he was turned over to the ER staff
without incident.

B22-03769 Threatening 05/13/2022 1529 Lancaster ST; On 5/13/2022 at approximately 1745hrs, I was notified by
Praxis Dispatch of a threatening call at Praxis, 1529 Lancaster St.,
Bluffton, IN 46714. Dispatch advised a male subject was
threating employees with a fork.
Myself and Officer Holliday arrived shortly before other

units. I observed an employee and a male subject sitting on
a bench outside the front doors. This subject was later
identified as Ramsay R. Brewer (DOB 10/18/1984).
The complaintant was reportedly not with the suspect,

so I asked the employee on the bench to open the door. He
quickly indicated that Ramsay was the suspect.
Other officers arrived and I stepped aside with the

employee to discuss the incident. He explained that
Ramsay had a metal fork and had been threatening to kill
other patients with it, and gouge or stab their eyeballs out.
After the fork was taken away Ramsay allegedly
commented that he'd still kill them with his bare hands.
Other officers informed me that Ramsay also attempted

to stab an employee with the fork. The fork/Ramsay's arm
was intercepted prior to the stabbing being completed.

On the listed date, officers were dispatched to 1529

Lancaster Street,
Praxis, for an unwanted party. Sgt. Archbold arrived at
Praxis, and as I was
west bound on Lancaster Street, I observed a black male
walking from the Praxis
area, east bound toward me, pulling along a suitcase. I
pulled over at Meadow
Lane as the male subject was walking toward me. I
stepped out of my squad
vehicle to speak with the subject. Officer Carlson and
Officer Belda arrived to
assist. The male subject gave to Officer Carlson his
identification card, after
locating it in his suitcase. I asked the male subject what
had happened and he
stated he was kicked out of Praxis. He stated he needed to
charge his phone but
they would not allow him to do so before leaving. It was
learned he had entered
a vehicle at Praxis, so I asked him if he was inside a vehicle
after being told
to leave. He stated he never entered a vehicle, but was
standing beside one.
He stated the door was just a touch ajar, so he closed it
completely. I
continued to speak with him, asking if he had entered the
vehicle, which he
continued to deny. After a bit of time, it was decided that he
would be given a
ride to Hardee's, where he could charge his phone, call
someone to come to
Bluffton to pick him up. After a pat down for officer safety, I
placed him in
my squad vehicle and transported him to Hardee's. The
business was closed so I
drove him to McDonald's. This ended my involvement with
the male subject.

Date 1/4/2022, Time, Reporting Officer: Detective Thomas
On 1/4/2023 I was asked by Chief Kyle Randall to take

over the investigation
regarding the missing report of Celeste Cuthbert. Celeste
had not been seen or
heard from since January 1, 2023 at 4:30 PM. She was last
seen leaving Praxis on
that date and time. I was advised that when she left she
was having a manic
episode and was extremely paranoid. I was originally
advised that, she was last
seen wearing an orange/red Spyder pullover with black
stripes by the sleeves,
black pants and black shoes and left the premises walking
eastbound on Lancaster
street on the north side of the road. It should be noted that
Celeste left with
only the clothing she was wearing. she had no cell phone,
no identification and
no money with her when she left.
I spoke with Celeste's fiance, Kristi Metzger, on the phone

on 1/4/23. Kristi
advised hat she had dropped Celeste off at Praxis on
December 29, 2022 and spoke
with her for the last time on January 1, 2023 at around 4:00
PM. Kristi stated
that Celeste was hallucinating due to her detoxing from
alcohol. Kristi stated
that she called back to Praxis, later that day, to check on
Celeste due to her
being concerned for her wellbeing but no one answered and
she left a voicemail.
Kristi stated that she called back on 1/2/23 to check on
Celeste and again got
no response and left a voicemail. Kristi stated that it was
not until she
finally spoke with someone at Praxis on 1/3/23 that she was
informed that
Celeste had left on foot on 1/1/23 at around 4:30 PM. Kristi
stated that she was
informed that Celeste was hallucinating and very paranoid
when she left the
facility. It should also be noted that Law Enforcement was
never notified by
Praxis of Celeste leaving even given her mental status at
the time. Kristi
stated that she received conflicting statements from staff
regarding when she
left, what door she exited and what she was wearing.
Kristi stated that she has reached out to all of Celeste's

family and friends
and no one else had seen or heard from her. I was able to

get Celeste's
property, including her phone, released it to her fiance and
emergency contact,
Kristi Metzger. Kristi was able to go through Celeste's
phone but stated that
there had been no activity on the phone since December
28, 2022 when she was
dropped off at Praxis.
Kristi stated that Celeste worked 2 hours on 12/28/2022

but asked Kristi to
pick her up because she was felling well. Kristi stated that
she took Celeste to
the hospital and she was later released. Kristi stated that
Celeste wanted to
get help for her alcoholism and it was her idea to go to
Praxis for treatment.
Kristi stated that Celeste is very family oriented and would
never intentionally
not contact her family knowing that they would be worried
sick about her. She
also stated that she has never been known to take off for
any amount of time
without someone knowing her whereabouts.
Kristi also voiced concerns that Celeste is not an outdoors

kind of person and
wouldn't have the capability to survive on her own in these
elements. Kristi
stated that Celeste knows absolutely no one in the Bluffton
area and only knows
a handful of people in Fort Wayne. Kristi reported that she
has never known
Celeste to use any illegal drugs other than marijuana
occasionally but she is a
very heavy drinker and suffers from alcoholism.
I made contact with ground searcher and K9 handler

Karen Karrer from the
National association of search and rescue. She responded
to the area with her
bloodhound Dudley and attempted to track Celeste's scent
from the door she
excited at Praxis on January 1st. Dudley led the team to
Lancaster Street and
the proceeded eastbound on Lancaster. The dog eventually
stopped following the
scent at a pond located at a home on Lancaster Street,
1317 Lancaster Street.
The first dog jumped into the pond and then sat down near
the edge of the pond.
The handler advised that this could mean that Celeste had
been or still is in
the pond. We searched the edge of the pond and the rest of
the property but it
was getting dark and we did not locate Celeste or anything
suspicious.
A second dog handler, Ed Ralston, and different

bloodhound arrived on scene
and was given a sample scent at Praxis. That dog also

tracked along the north

side of Lancaster Street heading east. The dog also
stopped and showed interest
at the same pond like the first dog and did not pick
Celeste's scent back up.
The second dog continued to go out onto the pier of the
pond and also appeared
to loose Celeste's scent.
We took both dogs to Handi Spot separately to attempt to

pick up a scent there
due to the high traffic of Praxis patients who frequent there
upon release.
Neither dog picked up Celeste's scent there. We also took
the dogs to Pak-A-Sak
and no scent was detected.
On 1/5/2022 Officer Carlson and myself went to the the

pond located at 1317
Lancaster. Officer Carlson was able to check the immediate
area with the
department issued drone while I checked the immediate
area on foot. Officer
Carlson did comment that he picked up a suspicious spot
with the drone on the
northeast corner of the pond approximately 8 feet from the
shore line.
On the morning of 1/5/2023 I met with several Officers

with DNR at the pond.
They were able to take a boat onto the pond with sonar and
they did pick up a
suspicious spot on the northeast corner of the pond at the
same location that we
noticed a suspicious spot under water with the drone.
DNR Officer Andrew Harmon suited up in his dive

equipment and dove to check
the suspicious spot. He advised that it was a very large pile
of rocks and he
did not locate anything else. He also swam and checked
the immediate area around
the pier and shore line and did not locate anything.
On 1/6/2023 officers from the Bluffton Police Department

went door to door in
the area and the extended area and handed out flyers and
asked citizens if they
had seen Celeste and received no new tips about her
whereabouts.
On 1/7/2023 we put together a search team and searched

a very large area on
foot and again with the drone and nothing was located.

Archbold's supplement regarding the exact locations
searched during this ground
search.
I also spoke with Jan Harkner with the Indiana Search and
response team about

the possibility of bringing a cadaver dog to the scene. Jan

advised that she
would be willing to do so but we needed a better area to
start the search. I
told her I would keep her in mind if we thought she could be
of help.
At this time a subpoena has been requested for Praxis to

gather further
information and I am waiting for it to be issued.

Narrative
Since the initiation of this missing persons report, I

(Archbold) have
participated in physical searches with others for the missing
person in this
matter, Celeste Cuthbert.
In the week following the report of the missing person,
searches
coordinated by Detective Clifford Thomas have included,
but were not necessarily
limited to a K-9 search on 01/04/22, spot checks by officers,
a door-to-door
canvasing with distribution of missing persons flyers on
01/06/22, and a search
party deployment on 01/07/22.
On the evening of 01/04, the department was assisted
by K-9 teams. The
K-9 teams indicated a sent track leading from the Praxis
facility to a residence
on W. Lancaster Street, Bluffton, where the track
terminated. This location,
1317 W. Lancaster Street, includes a residence,
outbuildings, and a pond.
Searches of property were permitted and included a
thorough search of the pond
by INDNR with sonar and a diver. The search yielded no
recovery.
On 01/06/22, patrol officers distributed approximately
one hundred
informational flyers to residences located on W. Lancaster
Street, Meadow Lane.
Virginia Court, N. Oak Extended, Hale Street and Wasau
Court. The flyers
included images and information, with a request for
property owners to search
their own properties (outbuildings, vehicles, etc.).
On 01/07/22, Bluffton Officers assisted by Bluffton Fire
Department,
completed a search of fields, wooded areas, ditches, and
fence rows. Searches
were done on foot and aided by ATV's and aerial drone.
This search covered the
area contained within the boundary of the SR124 bypass to
the west and north,
Lancaster Street to the south, and N. Oak Extended to the
east. Prior to the
search consent was obtained to search all areas within the
described perimeter
from the property owners or the property owner's agent.
Aerial drone also
completed surface searches over a few bodies of water.
These property searches
yielded no recovery or collection of potential evidence.

On Wednesday, 01/11/22, I walked a portion of a

railroad track to search. I
covered the portion of track bed from Dustman Road, south
to the Wabash River.
I have now covered the railroad from Hale Street to
Dustman Road. I also checked
a connecting portion of rail bed from Main Street to the
Wabash River.

On 1/9/2023 I received a subpoena signed by Circuit Court

Kiracofe for Praxis.
I hand delivered a copy of the subpoena to the facility
director Anthony
Phegley. I also spoke with Anthony and Celeste's therapist
Paul Painter in
private.
Paul stated that he had met with Celeste on 1/1/23 for a

session. He stated
that she was hallucinating, hearing voices and experiencing
tremors due to her
detoxing from alcohol. He reported that Celeste thought that
her sister and
brother in law were watching her with the facility cameras
and she was suffering
from paranoia.
Paul stated that Celeste was persistent about leaving the

facility and they
attempted to get her to stay. They stated that she eventually
exited the
facility out of a door on the east end of the building. Paul
could not give me a
definite clothing description on what Celeste was wearing
when she left. He
stated that he and another employee, Jenni Rogeness, did
follow Celeste out to
the intersection and attempt to get her to come back inside.
He stated that they
were both concerned due to her mental state and the cold
weather. He stated that
they were unable to convince her to come back inside and
she began walking
eastbound on Lancaster Street towards town. He stated
that this was at around
4:30 PM. He stated that he got off of work at 5:00 PM and
noticed Celeste
sitting on the curb at the intersection of Meadow Lane and
Lancaster Street.
Paul stated that he did not stop and speak with Celeste.
Paul did not have an
answer when he was asked why he didn't reach out to the
family to inform them of
the situation.
I also spoke with Jenni Rogeness in private. Jenni stated

that she unlocked
the door and reset the door alarm for Celeste and then
allowed her to leave. She
stated that she then walked through the building and exited
out of the front
door with Paul so they could speak with Celeste. She stated

that they caught up

with Celeste and spoke with her at the corner of the
property. She stated that
they could not convince her to come back inside and
Celeste was very standoffish
towards them. Jenni also stated that Celeste was paranoid
and suffering from
alcohol detox. Jenni stated that she was also concerned
about Celeste but made
no attempt to contact law enforcement or family.
I also received a tip from Jay Gerber stating that he

believes that he may
have seen someone fitting Celeste's description at the
Marathon (Airplane) gas
station in Bluffton. Jay was uncertain of which day it was but
stated that he is
in there most mornings between 5:00 AM and 6:00 AM. I
spoke with the Marathon
manager, Jenni Markley, and gave her a flyer and all the
information I had
regarding the tip. She is currently checking video
surveillance in an attempt to
find the female.
I also met with Kristi on 1/9/2023 and she had Celeste's

sister Celene on
speaker during the conversation. I was able to update them
on the investigation
and take down questions they had and things to follow up
on. I also retrieved
some personal items belonging to Celeste from Kristi
including a toothbrush, a
hairbrush, a watch and a bottle of lotion. These items will be
kept in an
attempt to retrieve Celeste's DNA if needed. Kristi advised
that Celeste is on
medicaid and I asked her to check with them and see if
there has been any new
charges or claims due to her being the emergency contact.
I also forwarded a flyer and Celeste's information to Wells

County Coroner
Hilarie Gaskill so she could send the information out to
other coroners in
surrounding counties just in case of a Jane Doe situation.
On 1/10/2023 I met with Anthony from Praxis and asked

him to show me the video
of Celeste leaving the building again. The video was from
1/1/2023 at
approximately 4:30 PM. In the video I observed Celeste
sitting in a chair near
the nurses station. An employee named Chris Koiner is
sitting in a chair next to
her but they do not appear to be interacting with one
another. Celeste then
stands up and begins walking down the hallway. She is
wearing what appears to be
a pair of light gray sweat pants, a light blue or gray hooded
sweatshirt, black

tennis shoes and a black coat. I asked Anthony why the

family was given a
different clothing description when they called and asked
what she was wearing
when she left. Anthony took full responsibility for that and
stated that was a
mistake on his part. Anthony stated that he looked on the
wrong date and on that
date Celeste was wearing the red Spyder pullover and
black sweats. Anthony
pulled up the video from the day before and it showed
Celeste wearing the red
shirt and black sweat pants.
When Celeste reaches the doorway on the video she is

approached by Jenni
Rogeness and Chris Koiner. Anthony stated that it is
protocol for 2 employees to
handle a situation like that together. He stated that one
employee will speak
with the patient while the other employee calls family.
Anthony believes that
Chris did not call family due to the fact that Celeste had just
spoken with
family on the phone. I have an appointment to speak with
Chris Koiner when he
returns to work on 1/13/2023
Updates were sent out informing everyone of the clothing

discrepancy.
Including 3 major news channels in Fort Wayne and the
departments Facebook page.
On 1/10/2023 the department also received a tip regarding

a jogger spotting a
black coat lying along the River Greenway against a tree.
The jogger believed
that she had seen the jacket a few days prior along with a
single red and black
glove. Officers searched the bank of the river on the
Greenway and also checked
the area with a drone but no jacket was located. A single
black/red glove was
located along the Greenway and photographed and
collected. The pictures were
shown to family members and they advised that they had
never seen that glove
before and claimed that Celeste did not take any gloves
with her when she
checked into Praxis.
On 1/11/2023 I met with Anthony Phegley again from

Praxis and our conversation
was recorded using an Olympus handheld recorder. I spoke
with Anthony again
about the camera footage and if there were any cameras
outside which he stated
that there wasn't. Anthony stated that he has also met with
the staff and
expressed the importance of family contact when a person

leaves against their

advice. He stated that he also told the staff to come forward
if they have any
information and that they need to cooperate with law
enforcement. I asked him
to show me the footage of Jenni and Paul going outside to
speak with Celeste
which he did. I observed Jenni and Paul exit the front main
door at around 4:40
PM and Paul reentered the front door at around 4:50 PM.
Anthony stated that, when a patient leaves the facility

against staff advice,
the staff is not allowed to help the patient in anyway. He
stated that if they
do it can result in immediate termination. Anthony stated
that the only staff
member that would have been leaving at the same time as
Celeste would have been
Paul Painter. He stated that most of the other staff's shifts
would have ended
later in the evening. Anthony stated that Celeste was the
only patient who left
the facility that day but they did have 3 patients check in on
1/1/2023 during
the 4:30 - 5:30 PM timeframe on that day.
Anthony stated that Celeste was prescribed medications

to help with her detox
as well as possibly for other treatments but he could not
disclose which
medications she was given. Anthony provided information
that I could give to
Kristi Metzger so that they could get the records released to
them. Anthony
stated that he did not have a lot of direct contact with
Celeste during her
stay. He stated that from what he observed she appeared
to be coherent but not
very involved within the facility. He stated that he was not
working on the day
Celeste left but from what he was told her mental state on
that day was not
consistent with how she had been previously acting. He
stated that alcohol
withdrawal is one of the worst detoxes you can experience
and she was definitely
acting out of character that day. This could have been a
combination of where
she was in her stage of detox and the medications she was
given.
Anthony stated that Celeste was offered the opportunity to

get her belongings
before she left but they could not convince her to come
back to the lobby to
retrieve them.
On 1/12/2023 I located a pile of clothing from behind gas

station. I observed
the clothing and went into the store to ask the employee if

he knew where they

came from. He followed me outside and stated that they
had been there for about
a week and he didn't know who they belonged to. The
clothing included 2 black
jackets, a couple of t-shirts and a miscellaneous make-up
items. II photographed
and collected the items and transported them to the Bluffton
pd. I sent the
pictures to Celeste's fiance and she stated that none of the
clothing belonged
to Celeste and that Celeste doesn't wear any makeup at all.
On 1/13/2023 I spoke with Praxis employee Chris Koiner.

Chris was the male
employee who assisted Jenni Rogeness with Celeste on
1/1/2023. Chris stated that
Celeste was upset and wanted to leave the facility after her
counseling session
with Paul Painter on 1/1/2023. He stated that he attempted
to have a
conversation with Celeste and he was trying to get her to
stay at the facility.
Chris reported that Celeste was very quiet and didn't seem
like she wanted to
converse with him at the time. Chris stated that he followed
Celeste and Jenni
down the hall to the door that Celeste eventually exited out
of. I asked Chris
why he didn't call Celeste's family while Jenni was talking
with Celeste. Chris
stated that he didn't call anyone because she had just
ended a phone call with
her fiance during her session with Paul. He stated that he
was told that Celeste
was already told by family that they would not come to get
her and she needed to
complete her treatment.
On 1/13/2023 I assisted a friend of Celeste's, Shawn

Richards, by offering
him areas that he could assist by searching with his drone.
To my knowledge he
was in town that day and did some searching. He used his
drone to search the
area around the white bridge and the area south of Praxis
around the animal
shelter, park and highway garage.
I have not received any new tips as of 1/16/2023 and the

investigation is
ongoing. I have spoken with Celeste's friends and/or family
several times in the
past week. No new information has been made available.
On 1/16/2023 Kristi Metzger sent me an email with

Celeste's medical records
that she was able to get from Praxis. I have attached a copy
of this to my
report.

Research and Response Team. It is attached as a file to

I also received a full medical report regarding regarding

Celeste's stay at
Praxis including the medications she was prescribed and
given while she was
there. I have attached it as a file to this report also.
On 1/18/2022 DNR did a complete grid search of the pond

located at 1317
Lancaster Street. They used 3 divers and I observed them
while they searched the
entire pond and nothing was located.
As of 1/26/2023 I have maintained contact with family but

no new leads or
tips have been received.

On 2/7/2023 the deceased body of Celeste Cuthbert was

recovered. At
approximately 1:30 PM we received a call from William
Britton with Safehands
field services. William advised that he believed that he had
located the dead
body of the missing person from Bluffton at a residence
located at 406 Meadow
Lane in Bluffton.
I responded to the address and spoke with Bill regarding

the discovery. Bill
advised that he works for a nationwide maintenance
company, that works with
banks, that specializes in restoring and preparing
repossessed and foreclosure
homes for resale. He stated that the home was secure
when he arrived on the
above date to clean up the backyard and place items into
the shed. He stated
that when he went to open the shed he observed Celeste's
body lying in between
the shed and a clubhouse in the backyard. Bill stated that
he had been to the
home a couple of weeks prior but the ground was covered
with snow and he did not
see Celeste's body at that time. He also stated that the
home was secure at that
time and it did not appear that she ever gained access into
the home.
Bill lead me to the body in the backyard and I immediately

believed the
decedent to be Celeste due to her clothing. I immediately
cleared the property
and called for a coroner and crime scene technicians from
the Indiana State
Police. I also obtained a search warrant for the property so
we could legally
conduct our investigation.
Wells County Coroner Hilarie Gaskill, Tim Dolby from the

State Police and
myself conducted an investigation of the body and the
scene. Tim Dolby
photographed the scene and the body, took measurements
and collected evidence.
There were no signs of foul play identified at the scene.

Celeste's coat was
located in the clubhouse along with 2 packs of Pall Mall
cigarettes in blue
packages. We also located 3 burnt cigarette butts matching

those cigarettes in
the clubhouse. Celeste was located lying on her back with
her arms out to her
side, palms down and her feet pointing to the west. We also
located a Mr. Frog
vape pen near her right hand with the name Celeste written
on it in Black
marker.
Jeff Lemler of Thomas rich and Lemler was called to the

scene to collect
Celeste's body and transport it to Fort Wayne for an
autopsy. The autopsy was
scheduled for 9:00 AM on 2/8/2023.
I made death notifications and answered question from

Celeste's sister Celene
Wright and Celeste's fiance, Kristi Metzger.
On 2/8/2023 I attended Celeste's autopsy. We were able

to further identify
Celeste by a list of tattoos that I had obtained from Kristi. I
am waiting on
the results from the autopsy which is pending at this time.
The pathologist
stated that he believes that her death was an accidental
death and that advanced
stages of alcoholism, the weather and her detox symptoms
likely contributed to
her death. No physical injuries were located or noted at the
autopsy.

I traveled to Lowell State Police Post and took custody of

sexual crime
collection kit, serial IN21-10539, PIN 1737. The kit was
transported from St
Catherine's Hospital Emergency Room by ISP Sergeant
Dwayne Dillahunty and
secured at the Lab at the post until I could get there to pick
it up.
I place the kit into evidence at the Bluffton Police

Department.

were calling about a collection kit in East Chicago for a

I spoke to Maricella who stated that she attempted to call

able to get the State Police to assist in collecting the kit and
