Conceived: August 2016.

Miscarriage : 31st October at 11th week of pregnancy.

Major Health Problems : Severe adenomyosis along with enlarged uterus and fibroid. Fallopian
tubes blocked, heavy bleading with clots and severe pain during menstrual cycle.

Medical History:

 I undergone through mayomectomy for the removal of uterine fibroid in November,

2012. (see file no. 1)
 I had a miscarriage at the 11th week of my pregnancy on 31st October and got DNC done.
(see file no.1)
 Later on i came to know after the HSG and USG done that my both fallopian tubes and
blocked and beginning of adenomyosis. (see file no. 1)
 I consulted a Dr. at GTB hospital, who suggested me for the laparoscopy for opening the
tubes. Several test were done. (see file no. 2)
 In February, 2018 I had undergone through a Hydro-tubation and Hysteroscopy to clear
the blockages in my tubes. Dr. has also prescribed me supportive medicine for conception
but I didn’t conceived. (see file no. 3)
 In August 2019, I consulted a Dr. in AIIMS , initially they suggested me for IVF but
later on after several test and MRI they advised me for adoption as conception is not
possible due to the critical condition of my uterus. (see file no. 4 and 5 (for MRI))
 I have attached my MRI and USG reports in file no. 5
 In august 2020 I went for IVF , Dr. clearly told me that due to my uterus condition
pregnancy will be risky and suggested me for surrogacy. I got my 1 embryo freezed but
could not get it implant till now. (see file 6)
 Since last 8 month I am under homeopathy treatment for the cure of adenomyosis but as
per my recent USG report the uterus condition has become worse.

I tried to compile all my prescriptions and reports as organized as possible. I have arranged them
date wise. Kindly refer the same.
