Memorandum 1

This is relative to your absences which brought to our attention. In particular, it appears that you failed to
report for work these past few days dated May 26 and 27 of 2023. It also appears that your absences was
not authorized under approved leave application nor communicated with the management about your
absences.

Record shows that this is not the first time you committed this kind of offense. Please be reminded that it is
also the duty and responsibility of the employee to inform the employer prior and during the leave of
absence.

In this regard, please be informed that unauthorized absence or absences without leave (AWOL) constitute
gross and habitual neglect of duties which is ground for termination under our Labor code.

Please be reminded that your employment status is still “PROBATIONARY” which implies that company
has the right to end the probation period immediately if the company finds the employee is either
underperforming in his job, poor time keeping or bad fit in terms of personality and organizational culture.

You are therefore instructed to explain in writing within (72) hours, why no disciplinary action shall be
imposed against you.

This Letter will serve as a WRITTEN WARNING LETTER to you. Repeated occurrences of similar
offense will result to more drastic disciplinary measures.
