Site Induction Course (SIC) Registration & Attendance Record

1. All persons who intend to work on this site shall attend the site’s in-house Site Induction Course (SIC) prior to commencing work.

2. Basic Lifting & Rigging VOC (HRW workers, Rigger/signalman, crane/lifting operators/managers/supervisor, excavator operator if doing lifting) should be completed before SIC.

3. SIC is scheduled in English on every Monday and Thursday from 0900 hrs - 1200 hrs. SIC in Mandarin is scheduled on every Friday. There will be no SIC training on Tuesday & Wednesday.

4. The subcontractor / company shall complete this SIC Registration form for their workers / staff at least 2 days in advance by 1500 hrs to Hexacon PIC. The cut off time for the registration will be at 1500 hrs to allow the safety
department to verify the documents submission before confirming the bookings date by 1700 hrs via email. We reserve the right to postpone the session if the number of attendees falls below 10.

5. Documents to be submitted are: Photocopy of ID (front & back), Apply Workplace Safety & Health in Construction Sites (AWSHCS) and competency certificates relevant to appointment on the site e.g. Rigger,
Signalman, Lifting Supervisor, Crane operator, Supervise construction work for WSH, Explosive power tool operator, Safety Coordinator, Scaffold erector, Scaffold supervisor, First aider, Driver’s licence,
Excavator operator certificate and etc. In addition, a Letter of Appointment or undertaking signed by the appointment holder must be submitted (use prescribed form). The approved signed copy of the
registration form with the attached documents will be returned to the Subcontractors to be produced on the day of the induction.

6. On the day of the Induction, the Guard shall tally the identity of the inductee with the registration form before issuing a Visitor Pass. All inductees must be escorted to the SIC Training Room by their inducted safety
supervisor, engineer or manager.
