WHEREAS, one of the pressing problems that the Municipal Department of Healh faces today is
the lack of ambulance vehicle that will be used in transporting patlents refened to Northem Samar Provincial
Hospital or other hospital in Tacloban or Manila;

IIUHEREAS, presenfly the Municipal Department of Healh has an existing 1 - Unit ambulance being
used but then it is not enough to cater he needs of he community and due to wear and tear he said unit
experiences mechanical problems;

WHEREAS, the request for additional I - Unit ambulance can be of great help in the promotion of
basic health services specially to the indigent constifuents who cannot afford to hire private vehicle in
transpoiling heh patients to the nearest hospital;

WHEREAS, it is but proper and fitting to look for any agency or individual who can extend help and
assistance in providing he said needs of tre Local Govemment Unit of San Roque.

NOW THEREFORE, upon motion of SB Member Alvin M. Lagrimas duly seconded by all he
members of fie Sangguniang Bayan present, be it...

Congresswoman Yedda Marie K. Romualdez, One Unit Ambulance Vehicle lntended for the Municipality of

RESOLVED FURTHER, that copies of this resolution be fumished and delivered to the 0ftce of
