The Domino theory
• Lack of management control – e.g. no structured training for slingers
• Individual and job factors – person slinging does not know safe technique • Unsafe acts and / or conditions – load exceeding SWL of sling is lifted • Accident – load falls and hits worker below • Loss – e.g. fractured leg or even fatality • statistically it is proven that a workplace fatality has many previous events before a worker is killed. • If these are not recorded they cannot be investigated and rectified. • If they are reported then solutions can be implemented at an early stage and therefore prevent the escalation to fatal accidents Reporting and Recording an accident
