I, GEORGE ELAIDA MORSO of legal age, Filipino citizen, a resident of Bldg. 9 Unit
303 Disiplina Village T. Santiago Street Lingunan, Valenzuela City, after having been sworn
to in accordance with law hereby depose and say:

1. That I am owner of a Professional Driver’s License with No. C09-20-004751

issued by Land Transportation Office;

2. That my former address on my Professional Driver’s License with No. C09-20-

004751 issued by Land Transportation Office was No. 9B Gemini St. Upper
Kalaklan, Olongapo;

3. That I am updating my former address in my Professional Driver’s License with

No. C09-20-004751 issued by Land Transportation Office to my new residence
address Bldg. 9 Unit 303 Disiplina Village T. Santiago Street Lingunan,
Valenzuela City;

4. That this Affidavit is being executed as the request of TRU/LTO to

update/change my address in Professional Driver’s License with No. C09-20-
004751 issued by Land Transportation Office and for whatever legal purpose it
may serve.
