AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, TEODORA D. DORATEO and LUCENO C. NOCELU, both of legal

age, Filipinos, and residents of San Fabian Street, Brgy. Tapalan, Salug,
Zamboaga Del Norte , after having been sworn to in accordance with law
hereby depose and say:

1. That we personally know JUAN P. DELA CRUZ of Brgy. Tapalan, Salug,

Zamboanga Del Norte;

2. That we know to best of our knowledge that JUAN P. DELA CRUZ died
on 10 January 2019 in Brgy. Tapalan, Salug, Zamboanga Del Norte due
to old age;

3. That the reason why there is no record of the fact of his death in the
Office of the Municipal Civil Registrar of Salug was because he died in a
far flung Barangay and his family immediately buried him without
reporting the matter to the aforementioned office;

4. That we personally know the foregoing facts because we are long time
friends and neighbors of JUAN P. DELA CRUZ and his family;

5. That we are not related to JUAN P. DELA CRUZ either by affinity or

consanguinity and has no interest in whatever this affidavit could be of
use either directly or indirectly;

6. That we execute this Joint Affidavit to attest to the truthfulness of the

foregoing statement and to support the application for delayed
registration of death of JUAN P. DELA CRUZ with the office of the Civil
Register of Salug, Zamboanga Del Northe, and for whatever legal
purpose this affidavit may serve.
