Risk Assessment CONTROL

All work assignments (site activities) should have a Specific Risk Assessment/Method Statement which
shall include specific attention to safety. "Follow-up" monitoring is required in order to prevent accidents.
Risk assessment will be carried out in accordance with the standard risk assessment template issued that
is issued along with the method statement template and adequately covering all specific environmental,
safety and health risks associated. The template is as follows:-

1.1. NUMERICAL RISK ESTIMATION TABLE

Least Consequences Most Consequences

Severity (S)

1 1 2 3 4 5
Least

2 2 4 6 8 10
Likelihood (L)

3 3 6 9 12 15

4 4 8 12 16 20
Most

5 5 10 15 20 25

• S = Severity of consequences should hazard occur (1 Least Consequence – 5 Most

Consequence)
• L = Likelihood of hazard occurring (1 Least likely – 5 Most likely)
• R = Risk = S x L (A Risk Rating of 1-25)

1.2. ACTION PRIORITIZATION TABLE

Risk Level (R) Assessment

1-2 Negligible

3-7 Acceptable providing risk is managed

8 - 12 Must be avoided if reasonably practicable
