SPECIAL POWER OF ATTORNEY

KNOWN ALL MEN BY THESE PRESENTS:

I, JOSELITO M. BORLONGAN JR., Filipino, of legal age, single and a resident of

No.112 Tinejero, Pulilan Bulacan, do hereby name, constitute and appoint Fernan Bantayan
Bermejo, of legal age, Filipino, and a resident of No. 2824 Aurora St. Rm. 303, FJ Bldg. Sta.
Cruz, Manila, to be my true and lawful attorney, for me and in my name, place and stead,
and to do and perform the following:

(a) To process, facilitate, and cause the correction of my birth certificate, and
for said purpose, appear before the Office of the Civil Registrar and all
other concerned government offices, agencies or instrumentalities;

(b) To secure copies or, if necessary, certified true copies, of my birth

certificate, baptismal, school, and other records from various government
offices, agencies and instrumentalities and private entities which maybe
required or needed for the purpose of correcting my birth certificate.

(c) To accomplish, prepare, execute, sign, file, and/or submit before said
offices all the forms and requirements needed, documentary or otherwise,
and to do any and all acts in furtherance of this Special Power of
Attorney.

(d) Should my said attorney deem necessary and convenient, to delegate, in

whole or in part, any or all powers herein granted or conferred in favor of
any other third person or persons whom my said attorney may select.

HEREBY GIVING AND GRANTING unto my said attorney full power and
authority to do and perform all and every act requisite or necessary to carry into effect the
foregoing authority as fully to all intents and purposes as I might or could lawfully do if
personally present, with full power of substitution and revocation, and hereby ratifying and
confirming all that said attorney or her substitute shall lawfully do or cause to be done by
virtue hereof.
