- Preventive maintenance services include necessary lubrication,
adjustments and diagnosis of unserviceable parts. Diagnostic services in
determining problem origin to include reading and interpreting the event
logs, testing and operation or diagnostic procedures until specific failing
component/units (whole or part of the Equipment is isolated.
- 2 times service interval per annum.
- Parts exclude consumable item such as AC & DC capacitor, Power Supply
Board, Batteries, Ventilation fans all external electrical cabling and electrical
components.
- Preventive Maintenance is not responsible for failure to render
maintenance services due to causes beyond its normal control including
and not limiting war, fire, flood, casualty, an act of God, strikes, Labor or
transport difficulties, unavailability of materials or services or interruption
of the electrical or other power
- Unlimited break down call and on site verification.
- 2 - 4 hours response time for failure.
- SLA: Next Business Day

EQUIPMENT SCHEDULE

- Inclusive of UPS component replacement and labour charges. Except
clause iii) as mention above.
- Type of Service: Comprehensive Preventive
- Nature of service: check & diagnose the Base unit, backup battery,
incoming supply, wiring connection, program, and setting.

One (1) unit of PICOBOX EMS System 1

Model: PICOBOX MM5K
- Type of Service: Basic Preventive Maintenance
- Exclusive of part replacement.
- Nature of service: Check & Diagnose, base unit, program and setting,
backup battery, incoming supply and SMS signal connectivity.

Total Price (based on Price Currency above) : 10,500.00

SST Payable (based on Price Currency above) : 630.00

Total Price inclusive SST : 11,130.00

TERMS AND CONDITIONS :

1. Payment All payments are due and payable 30 days from invoice date.
For Cash Before Delivery. Please bank to Maybank Berhad, Account No. 5122-3151-3944 and email the bank-in slip to us.
2. Price Validity Prices quoted herein shall be valid for 30 days and is subject to change without further notice while stock last.
Prices quoted are exclusive of any direct and/or indirect taxes that may be imposed by the relevant authorities from time to time,
which include but is not limited to any goods and services tax(GST) or similar taxes applicable to or imposed upon such supply of goods/services.
JOS reserves the right to modify pricing(including quotes) due to pricing or other error,currency and exchange rate fluctuations,
or any event beyond JOS's control, without liability to Buyer.
JOS shall reserve the right to revise the prices of goods and services quoted herein subject to the sales and services tax laws and the relevant applicable
that will be implemented
3. Delivery Estimated delivery time for Products: 6 – 8 weeks (subject to vendor's availability for goods) upon receipt of PO from Customer or signing of this Quotation, whichever is later.
4. Transportation Delivery is made at one location within 30KM of JOS Representative Offices located within Klang Valley, Penang and Johor Bahru. Additional charges
may be imposed for locations beyond 30KM radius.
5. Exclusion The price quoted does NOT include installation unless stated otherwise.
6. Cancellation NO cancellation is allowed once purchase Order is received by JOS.
7. Title & Goods Title to the Goods shall remain in JOS until full payment of Purchase Price is received by JOS. Risk shall pass to the Buyer upon delivery of Goods.
8. Others Purchases of the Goods are subject to JOS' General Terms & Conditions of Sale ("General T&Cs"), which can be found on JOS' website at the following
address: www.jos.com.my
9. Special Pricing Any Goods if sold to Buyer under special discount prices for the purpose of special project/deals are subject to special terms and conditions
imposed by manufacturer of the Goods. JOS reserves the right to recover from Buyer any discounted sum and the Buyer shall indemnify JOS against
any losses and damages suffered by JOS or claims from any third party as a result of Buyer's breach of any of such special terms and conditions.
10. Export Compliance Unless expressly exempted by JOS in writing, Buyer (i) undertakes not to, (ii) shall ensure that persons under its control shall not, and (iii) shall use
all reasonable endeavours to ensure that any persons Buyer resells the Goods shall not, directly or indirectly export the Goods from Malaysia, resell
the Goods in any other country and/or resell the Goods to a customer where the effect of such sale would result in the Goods being located or sold
to a location outside Malaysia. If Buyer is an end user who had purchased the Goods for its own use, Buyer undertakes to use the Goods in Malaysia
and/or any other territory agreed in writing by JOS.

By signing this quotation, Buyer confirms the purchase of all the items quoted and their prices listed above. Buyer also confirms it accepts the General Sales T&Cs and all terms and
conditions contained herein, to which Buyer hereby agrees that this quotation and any purchase of Goods herein provided are subject to.
A separate purchase order issued by me/ our Company is NOT required. Please proceed with the delivery.
